Were looking for a compassionate, dependable Hospice LPN to join our team and provide exceptional, person-centered care in the community.
What Youll Do
- Provide skilled nursing care to hospice patients in their homes and other care settings.
- Monitor patient conditions and communicate changes to the hospice care team.
- Administer medications and treatments as directed by the plan of care.
- Educate and support patients and families with compassion, patience, and respect.
- Work collaboratively with RNs, providers, social workers, chaplains, aides, and other members of the interdisciplinary team.
- Help patients remain comfortable and supported while honoring their goals and preferences.
- Complete accurate clinical documentation and follow hospice standards of care.
What Were Looking For
- Current LPN license in Maine.
- Strong clinical skills and sound judgment.
- Excellent communication and organizational skills.
- A compassionate, respectful approach to patients and families.
- Ability to work independently while being an engaged member of a collaborative team.
- Hospice, home health, or palliative care experience is valued, but were also interested in LPNs who are passionate about learning hospice care.
