About this role

The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a skilled nursing facility under the supervision of a Registered Nurse (RN) or Director of Nursing. The LPN is responsible for administering medications, monitoring resident health, assisting with care plans, and ensuring high-quality care in accordance with facility policies and state regulations.

Key Responsibilities

- Provide direct nursing care to residents in accordance with care plans and physician orders.

- Administer medications and treatments as prescribed.

- Monitor residents’ vital signs, conditions, and responses to treatment.

- Report changes in resident condition to the Registered Nurse or physician.

- Assist with the development and implementation of individualized care plans.

- Supervise and support Certified Nursing Assistants (CNAs).

- Maintain accurate and timely medical documentation.

- Ensure compliance with infection control and safety standards.

- Respond to resident needs and emergencies.

- Participate in shift reports and interdisciplinary team meetings.

- Provide compassionate care while maintaining resident dignity and privacy.

Qualifications

Qualifications

- Current Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) license in the state of employment.

- Graduate of an accredited practical nursing program.

- Experience in long-term care or skilled nursing preferred.

- Current CPR certification required.
