Are you a people person who loves connecting the dots? Do you enjoy solving problems, building relationships, and helping people navigate systems that can sometimes feel like a maze?

Were looking for a Case Manager who is compassionate, organized, resourceful, and ready to roll up their sleeves to help clients reach their goals.

As a Case Manager, youll be the connector between clients, families, treatment teams, schools, community resources, and service providers —keeping everyone moving in the same direction and, most importantly, keeping the clients needs at the center of the conversation.

What Youll Do

🧭 Help Turn Goals Into Plans

- Work with clients to identify short- and long-term goals and the steps needed to reach them.

- Help clients build the skills and confidence they need to succeed at home, school, work, and in the community.

- Assess client needs on an ongoing basis and help determine the appropriate level of care and services.

- Develop and coordinate individualized service plans with the client, family, and treatment team.

- Keep discharge and transition planning in motion from intake all the way through discharge .

🤝 Be an Advocate & Connector

Youll help clients and families navigate the world of services and resources by:

- Connecting clients with community resources, natural supports, and formal services.

- Building strong professional relationships with youth, families, schools, providers, and community partners.

- Serving as a liaison between referral sources, external partners, and internal Lighthouse Youth & Family Services programs.

- Helping clients access the support they need to increase independence and strengthen daily living skills.

- Advocating for clients and families through direct service, case management, crisis support, and appropriate therapeutic interventions.

🧠 Help Navigate the Tough Stuff

Some days will be straightforward. Others may require creativity, patience, and a really good game plan.

Youll:

- Support crisis management and stabilization when needed.

- Help clients identify and address symptoms, behaviors, and thought processes that may create barriers to education or employment.

- Provide mental health prevention services in outpatient and/or school-based settings.

- Help clients and families problem-solve when life gets complicated.

- Stay calm, focused, and trauma-informed when situations become challenging.

📋 Keep the Details Moving

We know paperwork isnt always the glamorous part—but good documentation helps make good care possible.

Youll:

- Complete progress notes and service documentation on time.

- Document and submit required billable service units.

- Maintain client files according to regulatory and agency standards.

- Initiate services within required agency timeframes.

- Prepare reports, service plans, discharge plans, and case closure documentation.

- Keep information organized and communicate important updates to the treatment team.

🚀 Be the Teams Connector

Youll collaborate with clinical and support teams to make sure clients receive quality, consistent, coordinated care .

That means communicating, collaborating, attending meetings, completing training, and keeping your professional skills sharp.

And the relationship doesnt necessarily end at discharge—youll provide after-care supportive services through monthly phone contact for six months following discharge .

Youll Be Great in This Role If You Are

- Trauma-informed and able to meet clients where they are.

- A strong communicator , both in conversation and on paper.

- A thoughtful problem-solver and critical thinker .

- Organized enough to keep multiple moving pieces on track.

- Comfortable assessing needs and coordinating services.

- Familiar with health, mental health, and social service systems—or excited to learn them.

- Comfortable using Microsoft Office and case management software.

- Someone who can build professional relationships with clients, families, providers, schools, and community partners.

- Flexible enough to know that sometimes the plan needsa new plan.

Qualifications

What Youll Need

Required:

- Bachelors degree in human services or a closely related field OR a high school diploma/associate degree with three years of relevant experience.

- QMHS certification required.

- Excellent interpersonal and written communication skills.

- Demonstrated computer proficiency.

Preferred:

- Experience providing mental health case management services.

- CDCA credential.

Where Youll Work

Depending on the service line, this role can look a little different.

🏫 Community & School-Based

Youll have a professional office home base, but your work will also take you into client homes, schools, and the community .

This position offers hybrid work options, combining remote and in-office work. Specific in-office days will be coordinated with your team and the needs of the business and may change based on program needs.

🏠 Shelter-Based

Youll work from a professional office environment and/or satellite offices, with hybrid work options that combine remote and in-office work.

This Isnt Just a Desk Job

Youll regularly communicate with clients, families, coworkers, and community partners. Youll also spend time standing, walking, handling objects, and reaching with your hands and arms.

Occasionally, you may need to lift or move items weighing up to 30 pounds .

Schedule & Flexibility

Most hours fall within the standard operating hours of your assigned service line. Occasionally, the schedule may include:

- Evenings

- Weekends

- Holidays

Because client needs dont always check the clock before showing up.

Hit the Road 🚗

Travel is primarily regional during the week, although some out-of-area travel may be required based on client needs.

Youll also be expected to transport clients in a personal vehicle.
