About the Role
- Community-based physician rounding in subacute skilled nursing facilities
Flexible scheduling — you control your day
- Low census: 10–20 participants seen once weekly
- Lead clinical oversight and quality for PACE provider services
- Collaborate with an experienced, supportive interdisciplinary team
- Serve as a physician leader and mentor to NPs and other providers
- Champion person-centered care for frail elderly participants
Compensation & Benefits
- Competitive physician salary — commensurate with experience
- $5,000 annual CME allowance
- Only 2 call days per month

Qualifications
- MD or DO; licensed to practice in Colorado
- ABMS Board Certified — Internal Medicine or Family Medicine (geriatrics preferred)
- DEA registration; ability to maintain hospital privileges
- Experience in primary care with frail or elderly populations
- Passion for mission-driven, whole-person care
