About this role
The Palliative Care Social Worker is part of our interdisciplinary Home-Based Palliative Care team and supports patients with serious illnesses and their families through advance care planning, psychosocial assessment, care coordination, and connection to community resources. This role focuses on improving quality of life, reducing avoidable hospitalizations, and helping patients remain safely at home. Qualifications: Master's Degree in Social Work (or related field) and Ohio LSW required. LISW preferred. Healthcare, community social work, care management, or case management experience preferred. Position is Monday-Friday, no weekends or holidays. Position requires some local travel to patients homes and facility settings.
