🌿 What Youll Do
As an RN Case Manager , youll coordinate and oversee the interdisciplinary plan of care for an assigned caseload of hospice patients, ensuring they receive compassionate, high-quality care wherever they call home.
Youll serve as a key clinical partner for patients, families, physicians, and the interdisciplinary hospice team throughout the patients hospice journey.

🩺 Key Responsibilities
- 💙 Manage an assigned caseload of hospice patients and families.
- 🩺 Complete comprehensive patient assessments and ongoing evaluations.
- 📋 Develop, implement, and update individualized plans of care.
- 💊 Support effective pain and symptom management.
- 🤝 Coordinate care with physicians and members of the interdisciplinary team.
- 🕊️ Provide compassionate education, guidance, and emotional support to patients and families.
- 👩⚕️ Supervise Hospice Aides and Licensed Practical Nurses as appropriate.
- 💻 Maintain accurate, complete, and timely electronic medical record documentation.
- 📞 Communicate changes in patient condition and care needs to the appropriate clinical team members.
- 🌱 Participate in interdisciplinary meetings and quality improvement initiatives.
- 🏠 Provide patient-centered hospice care wherever patients call home.

Qualifications
🎓 Qualifications
Were Looking for Someone Who Has:
- 🩺 A current, valid Florida Registered Nurse (RN) license .
- 🏥 A minimum of one year of inpatient nursing experience OR previous hospice, home health, or long-term care experience .
- 💬 Excellent communication and relationship-building skills.
- 🧠 Strong clinical assessment, critical-thinking, and problem-solving abilities.
- 🤝 The ability to collaborate effectively with physicians, patients, families, caregivers, and members of the interdisciplinary team.
- 📋 Strong organization and time-management skills with the ability to manage an assigned patient caseload.
- 🚗 A valid Florida drivers license , reliable transportation, and current automobile insurance.
- 💙 A passion for serving patients and families with compassion, dignity, and professionalism.
