JOB SCOPE:
This is a direct service position requiring specific knowledge of mental health, and skills in crisis
intervention and stabilization. The Transition Specialist will assess individuals for transition readiness,
provide linkage and escorts to appropriate outpatient providers as well as provide post discharge followup.
As part of the ACT you must be willing to go out into the community to provide services. This
position requires clinical experience, and the desire to work as part of a team.
ESSENTIAL FUNCTIONS:
Conducts daily visits in the community, and subsequent progress notes and other required
documentation in accordance with agency and OMH standards.
Completes Wellness Plans, Comprehensive assessments and other documentation as required.
Complete Transition Checklist with all consumers at various stages of treatment, immediate
needs and recommend treatment objectives for the consumers service.
Participates in daily morning meeting, and inform team in daily meetings and weekly clinical
meetings around consumers ready to transition off the ACT team.
Collaborate with outside community referral agencies, providers and Care Coordination services
for aftercare, to build referral options for transiting consumers.
Provide 24-hour crisis intervention on-call services on rotating basis.
Performs other duties, consistent with the goals and objectives of the program, as directed by
Team Leader.
Qualifications
EDUCATION AND EXPERIENCE:
Bachelor Degree required, Masters Degree preferred - LMHC, LMSW, LCSW, or equivalent in licensure in NYS.
At least one of experience working in human services with individuals with serious mental illness and experience in work with families.
