The LPN works under the direction of a Registered Nurse to deliver patient-centered care, support treatment plans, monitor patient conditions, and assist with admissions, discharges, and patient education.
Position Details:
Key Responsibilities
- Provide direct patient care within the LPN scope of practice.
- Assist with patient admissions, discharges, assessments, and care planning.
- Monitor patient conditions and promptly communicate changes to the RN.
- Participate in bedside shift reports, patient rounding, and interdisciplinary collaboration.
- Reinforce patient and family education.
- Delegate and support patient care activities for unlicensed assistive personnel as appropriate.
- Maintain accurate documentation and follow hospital policies and procedures.
- Complete required competencies, certifications, and ongoing education.
- Contribute to quality improvement initiatives and department goals.
Qualifications
- Graduate of an accredited Practical Nursing program.
- Current LPN license in the state of practice.
- New graduates and experienced LPNs encouraged to apply.
- Strong communication, teamwork, and patient care skills.
Certifications
- BLS (American Heart Association) required within 30 days of hire.
- IV Certification required upon hire or within 3 months of hire, based on department requirements.
- Additional certifications (ACLS, PALS, PEARS, S.T.A.B.L.E., or other unit-specific certifications) may be required based on assignment.
