About this role

- Identifies patient mental health problems and prescribes treatment.

- Implements mental health services utilizing therapeutic regimens.

- Provides mental health education to patients and their families.

- Provide health/behavioral health assessment (including differential diagnoses, prognosis), treatment recommendations and treatment planning for patients across program types.

- Provide medication management for patients on medication and intermittent reassessment (as needed) for those assessed (but not on medication).

- Provide counseling, brief therapy, psychoeducation about behavioral health and other care for patients as planned by the treatment team and to support the treatment plan of the client.

- Participate in case reviews and contribute to treatment planning goal setting.

- Provide consultations to non-medical staff on complex cases.

- Consult with primary care provider, substance abuse counselors, blended case managers, and other mental health professionals as needed.

- Maintain professional behavior and relationships with all Friendship House staff and referral resources.

- Attend staff meetings, provider meetings and other meeting as required.

- Fill out and maintain patient-related paperwork, including federal- and state-mandated forms.

- Participate in assessments and medication management off the main site as needed.

- Provide crisis intervention, emotional support, resources and referrals as needed.

- Comply with Agency policies and procedures, including those regarding conduct, confidentiality, and record keeping.

- Performs in-person and/or telehealth services to identified populations either at the remote location or in the office as directed by agency requirements and policies.

- Perform duties in a professional manner in response to clients, co-workers, and situations in which own values are challenged.

- Other duties as assigned.

Qualifications

- Board Certified or Eligible in Nurse Practitioner

- Current licensure in Pennsylvania

- 2 years’ experience working in mental health setting.
