Helping Hands Outreach Worker

Reports to: Director of Mental Health, Mental Health Department

35 hours per week

One year contract

POSITION SUMMARY

The Helping Hands Outreach Worker will help individuals who are facing homelessness or at risk of becoming homeless in the community of Ojibways of Onigaming and their members who live off the community.  They will reach  out to individuals who are experiencing homelessness or about to become homeless to provide life saving supplies and access/transportation to safe indoor space.  They will assist community members in finding and keeping housing.  The worker will work with community members to develop and achieve housing plans, provide assessment, informal support, linkage and accompany them to housing and other needed services.

ROLES AND RESPONSIBILITIES

- Build trusting, respectful and supportive relationships with Ojibways of Onigaming members through regular engagement and outreach.

- Provide one on one support and guidance to individuals facing homelessness or to individuals who are at risk of becoming homelessness.

- Develop and deliver educational workshops, life skills programs, promote healthy living, awareness and engagement

- Assess the needs of Ojibways of Onigaming and identify barriers to access and determine the best ways to provide support

- Advocate for the vulnerable population while maintaining professional boundaries, confidentiality and respect for privacy.

- Participate in case planning,

- Help individuals access social services, health care, community programs

- Maintain accurate documentation, statistics, reports and confidential client records as required,

- Participate in ongoing training, professional development and department meetings

- Support community events and wellness initiatives that promote healthy, substance-free lifestyles

- Collaborate with other programs in the community

QUALIFICATIONS

- An equivalent combination of education, relevant work experience, cultural knowledge, and lived experience will be considered

- Diploma or Degree in Social Work, Human Services, indigenous studies, Psychology or a related field

- Experience working with indigenous populations and homeless population

- Knowledge and appreciation of indigenous culture, traditions, language, and community-based healing approaches

- Strong interpersonal, communication, facilitation and conflict resolution skills

- Ability to build positive relationships with individuals while maintaining professional boundaries

- Valid Ontario class G driver’s license and access to reliable transportation

- Current First Aid and CPR certification or willingness to obtain

- Satisfactory Criminal Record Check with Vulnerable Sector Check
