Care Manager Social Worker, LBSW or LMSW

Social Worker Care Management | Full-time, 80 hours bi-weekly
Monday-Friday, 8a-4:30p, exempt

Meritus Health is seeking a compassionate and collaborative Social Worker to join our Care Management Team. This role supports patients throughout their acute care episode and ensures smooth transitions post-discharge. Key Responsibilities:

- Coordinate psycho-social support for patients during hospitalization

- Develop and implement personalized discharge plans

- Assess patient needs related to care coordination, resources, and services

- Collaborate with physicians, nurses, and care coordinators to enhance patient satisfaction

- Facilitate access to community resources to support recovery and continuity of care

- Ensure compliance with hospital policies, regulatory standards, and evidence-based practices

Qualifications: Education:

- Bachelors degree in Social Work required

- Masters degree preferred

Experience:

- Minimum two years in acute care, SNF, physician offices, or community health settings

Licensure/Certification:

- LBSW or LMSW in Maryland required

- Must obtain care/case management certification within five years of hire

Skills & Abilities:

- Strong communication and multitasking skills

- Familiarity with EMRs and ability to navigate electronic medical records

- Knowledge of disease processes, regulatory requirements, and evidence-based care

- Understanding of community resources, provider networks, and the revenue cycle

- Ability to build relationships across clinical and community settings
