Job Summary
Provides support in planning, directing and coordinating care of patients. Assists in obtaining patient medical records, schedule patient diagnostic testing, contact patients by phone and/or written correspondence. Writes letters, mail and fax documents. Develops and maintains organized records and documentation.
Essential Functions

- Requests and obtains outside medical records from various providers and/or facilities
- Schedules initial and follow up appointments.
- Contact patients by phone, MyChart, written correspondence regarding appointments
- Faxes or mails medical information to case managers, referring physicians, dialysis units and consulting physicians
- Performs other duties as assigned.
Supervisory/Management Responsibilities
- This is a non-management job that will report to a supervisor, manager, director, or executive.
Minimum Requirements
- Education - High School diploma or equivalent. Associate degree preferred
- Experience - Two (2) years related experience

- Advanced ability in use of electronic medical record, computer software, correspondence skills, maintenance of database
- Communication skills.
- Knowledge of medical terminology
