The Director Long Term Services and Support is an integral member of the Select Health South Carolina Medical Management Team.
Work Arrangement:
- Fully Remote
- Must Reside in South Carolina
- Travel throughout the state will be required
Responsibilities:
- Serve as internal consultant to Case Managers, Medical Director, and Provider Network Contracting regarding Director Long-Term Services & Support (LTSS) services and providers in the State of South Carolina.
- Serve as subject matter expert for LTSS training for internal care teams and external audiences.
- Support coordination of LTSS with other clinical and social services across the continuum of care striving to enhance the quality of member care, including participation on multidisciplinary care teams as indicated.
- Accountable for maintaining operational processes, policy and procedures to support LTSS care delivery.
- Provide oversight of care team’s use of State’s Phoenix system for planning and management of Home and Community Based Services.
- Assume lead role in phasing-in state’s Home and Community Based Service system roles and responsibilities, as articulated in MMP PRIME MOU and Three Way Contract.
- Ensure quality and performance metrics are consistently met regarding LTSS, as specified in MMP PRIME MOU, Three Way Contract and quality performance plan.
- Be the primary source of contact with South Carolina Department of Health and Human Services regarding LTSS.
Education/Experience:
- Bachelor's Degree with 5 to 10 years' experience.
- Previous management experience including hiring, training and performance management.
- Thorough knowledge of Long Term Care and Home and Community Based Services (HCBS) guidelines, rules and requirements.
