The organization operates 805 long-term care beds and 308 seniors’ apartment units, with an additional 224 long-term care beds opening in a new home by June 2026. In addition, Yee Hong offers four campuses of managed care, a hospice residence, and a wide range of community-based senior and caregiver support services. Through this comprehensive network, the organization serves more than 15,000 individuals annually across the Greater Toronto Area.

Yee Hong is preparing to welcome residents to the   Ng Siu Chan Care Centre , a new, purpose-built home that will provide 224 seniors with high-quality, culturally responsive care in a modern and welcoming environment.   We are creating a place where seniors can feel   at home, understood, respected, and cared for .

Opening in   early 2027 , the Ng Siu Chan Care Centre   represents   an exciting new chapter for Yee Hong. The home focus es   on serving the Chinese community and providing care that reflects residents’ language, traditions, food, programs, activities, and unique cultural needs.

Job Description:

Reporting to the   Director of Resident Care , your main   purpose   will be:

- To   assist   in auditing care plans and ensure consistency and completeness   in accordance with   the   organization's   resident- centered   approach, relevant government standards, residents'   rights,   and the organization’s mission,   vision,   and philosophy.

This role plays an important part in supporting the nursing team by ensuring documentation and care plans are carefully   maintained   and   reflect   each resident’s needs. By combining attention to detail with a commitment to resident- centered   care, you will help strengthen quality, continuity, and safety of care - ensuring the right information is available to support the best possible outcomes for every resident.

What You Will Do:

- Provide efficient and reliable clerical support to registered staff and nursing management, primarily in residents' documentation.

- Assists   in auditing care plans to ensure consistency and completeness .

- Inputs and   maintains   accurate   data,   statistics   and client information   in accordance with   the LTC Home requirement .

- Keeps   up-to-date   with development and best practices in the field .

What You Will Bring:

- Completion of 2nd year of a RN program or   above;   Or c urrent enrollment in an RN   program.

- Familiarity with medical terminology .

- Ability to communicate in Chinese or Japanese is an asset.

- Satisfactory Police Vulnerable Sector Screen .
