About this role

Overview

As an In Home Aide with Your Home Court Advantage, you will play a crucial role in enhancing the lives of our patients by providing assistance with daily living activities and ensuring their comfort and well-being. You will work closely with our healthcare team to deliver personalized care tailored to the unique needs of each individual.

Responsibilities

- Personal Care as needed

- A successful Home Health-care Aide provides personal care services under the direction and supervision of the Registered Nurse

- Ability to follow a care plan

- Meal preparation/feeding

- Laundry

- Light Housekeeping

- Knows and follows infection control policies including proper hand washing techniques.

- Reports any observed or reported changes in the client’s condition and/or needs to the Registered Nurse.

- Documents care provided and completes the forms required for the client’s records. Completes the appropriate records to document cares given and pertinent observations.

- Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager.

- Demonstrates safe practice in the use of equipment. Does not use equipment until orientation has been provided. Notifies supervisor of educational needs.

- Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.

- Treats patients and families with respect. Maintains confidentiality in all aspects of the job.

- Accepts and fulfills assignments with the agency. Exercise good judgment in accepting assignments.

- Participates in required in-service programs to meet compliance requirements. Identifies learning needs to agency supervisor.

- Performs other related duties and responsibilities as deemed necessary

Qualifications

- Meet the training and/or certification requirements of the state in which the home health aide services are being provided. This includes specifications of contracted agencies for which services are being provided.

- Active and verifiable State Tested Nursing Assistant (STNA) or Certified Nursing Assistant CNA certification in applicable state OR 1 year of supervised, verifiable experience within the last 3 years.

- Be at lease eighteen (18) years of age.

- Demonstrated ability to read, write, and follow a written Plan of Care.

- Good interpersonal skills and a passion for caring for those in need.

- Current driver’s license, good driving record, and/or reliable transportation is required.

- 1 + years of experience is highly preferred.

- Frequent standing walking, stopping/bending infrequent squatting/kneeling and balancing

- Weight handled: push, force required: 30 - 50 lbs (at times)
