Personal Care Clinical Coordinator at Landis Homes Personal Care

Position Essential Duties/Responsibilities:

- Works closely with the Director of Personal Care, day shift charge nurses and alternate shift RN Campus Supervisors for effective 24-hour management.

- Recognizes and responds to significant changes in resident condition and provides leadership in any emergency.

- Rounds daily on each house, ensuring level of care needs are met.

- Provides clinical support to the team members.

- Oversees quality issues, monitors incidents/accidents, provides follow-up monitoring, and addresses issues immediately.

- Works with RH Infection Control Preventionist in monitoring infection control issues in assigned house(s), completes forms and reviews reports.

- Assists with direct care, such as bathing, assists in the dining room, responds to call bells and participates in resident activities as able.

- Serves as a change agent, communicating change positively to staff.

- Is available (on-call) in an emergency situation when required staffing is affected.

- Shares weekly on-call with the Director of Personal Care.

- Works 2 shifts per month on the floor as a nurse.

- Works with scheduler on staffing issues to ensure that call-offs and vacancies are filled to provide adequate coverage as per regulatory requirements and resident requirements.

- Coordinates monthly billing assessments with Personal Care households,and submits billing to the Finance department.

Qualifications

LPN Qualifications

- Nursing/healthcare experience required

- Licensed in the state of PA

- Current CPR certification
