Medical Case Manager - Ryan White (Job Overview)
Schedule: Monday-Friday (8AM-5PM)

As a Medical Case Manager under the Ryan White Program, you'll be pivotal in supporting and guiding our HIV+ patients through their healthcare journey. You'll provide vital psychosocial services and become a champion of patient advocacy, ensuring that clients have access to the resources they need to thrive.

Key Responsibilities
- Identify and screen HIV+ individuals referred by Legacy providers to assess social service needs.
- Provide information, referrals, and assistance for medical, psychosocial, and basic needs services to enhance patients' independence and health.
- Advocate proactively on behalf of clients to minimize service gaps and eliminate barriers.
- Support patients who have fallen out of care in reconnecting with medical services.
- Adhere to care standards including DSHS Texas HIV Management Standards.
- Complete Comprehensive Assessments and Individualized Care Plans with patients to tailor their support.
- Ensure proper documentation for program standards within the required time frames.
- Engage in continuous learning through trainings and community meetings.
- Monitor service efficacy with peers and supervisors through evaluations.
- Demonstrate respect and foster positive relationships with patients and team members.
- Work effectively within a team-based approach to healthcare, promoting timely access and cultural sensitivity.
- Travel between locations as needed, including between Houston and Beaumont.
- Perform additional duties as assigned, contributing to the broader mission of Legacy Community Health.
Minimum Qualifications
- High School diploma or equivalent required.
- One year of case management experience with people living with HIV and/or persons with a history of mental illness, homelessness, or chemical dependence is required.
- All licensed professionals must maintain their professional licenses as define by their respective Texas licensing/examining boards.
- A degree in health, human or education services is strongly preferred.
- Excellent communication skills and customer service skills.
- Requires a working knowledge of community resources and treatment issues related to HIV.
- Must be able to manage multiple deadlines and prioritize.
- Demonstrated competency with an EHRS system.
- Ability to multitask in a fast-paced environment.
- Above average organizational skills.
- Strong typing and computer skills.
- Must be able to travel between locations as needed, including the ability to travel between Houston and Beaumont.
- This job requires access to confidential and sensitive information, requiring ongoing discretion and secure information management.
- Bilingual (English/Spanish) is preferred.
- If the employee is bilingual, then they must have written and verbal fluency in English and Spanish.
- Experience working among the gay, lesbian, bisexual and transgender community is preferred.
