Accountabilities
- Perform remote medication order entry, order verification, and drug verification across multiple hospital environments.
- Complete transitions-of-care medication reconciliation and navigate multiple pharmacy information systems.
- Provide drug information and therapeutic guidance to physicians, nurses, and other healthcare providers.
- Assess medication appropriateness, dosing—including renal and hepatic adjustments—drug interactions, and medication compatibilities.
- Support diverse and specialized patient populations, including pediatric, geriatric, pregnant or nursing patients, patients with HIV, and those with renal or hepatic impairment.
- Apply clinical knowledge across IV therapies, TPN, cardiac and narcotic drips, IV antibiotics, chemotherapy, and other complex medication therapies.
- Accurately and promptly document clinical interventions, medication reviews, and other pharmacy activities.
- Maintain compliance with HIPAA, applicable pharmacy laws, client policies, and professional standards.
- Work effectively across VPNs, automation systems, pharmacy information systems, and productivity tools such as Google Workspace, Microsoft Office, and Snagit.
- Maintain required pharmacist licenses, credentials, system access, and ongoing professional competencies.
- Participate in clinical, system, and policy training while consistently meeting established productivity and quality standards.
Requirements
- Bachelor’s degree in Pharmacy required; PharmD preferred.
- At least 1 year of hospital pharmacy experience strongly preferred, ideally in a clinical or acute-care environment.
- Active pharmacist licensure in Illinois or California in good standing is required, with willingness to obtain additional licenses in Iowa, Nebraska, and Hawaii as needed.
- Strong clinical knowledge and the ability to evaluate medication safety, dosing, interactions, compatibility, and therapeutic appropriateness.
- Comfortable working across multiple pharmacy information systems, technology platforms, and remote clinical workflows.
- Strong communication skills with the ability to collaborate effectively with physicians, nurses, pharmacists, and other healthcare professionals.
- High attention to detail, sound clinical judgment, and the ability to work independently in a technology-driven environment.
- Willingness and availability to work overnight shifts, weekends, holidays, and a 7-on/7-off schedule.
- Reliable home internet connection through cable broadband or fiber optic service with minimum speeds of 10 Mbps download and 5 Mbps upload.
- Ability to maintain all required professional credentials, licenses, and system access throughout employment.
