JOB TITLE : Licensed Practicing Nurse (LPN)

REPORTS TO : Director of Assisted living Services

SALARY CLASSIFICATION : Hourly

Under the direction of RN, has thorough knowledge of principles, methods and techniques involved in performing general nursing duties. Ability to evaluate change in condition, initiate and maintain ongoing medication management and respond to all emergency situations within the guidelines of good nursing practice.

Essential Job Functions :

- Participating in the process of ALSA admissions, readmissions and change in condition management.

- Initiate and maintain residents medication management needs.

- Demonstrates the ability to evaluate people, situations, relationships.

- Demonstrates resourcefulness and ingenuity.

- Demonstrate the willingness and ability to support and supervise the line staff

- Develop and maintain effective communication skills with MD, APPRN, nursing staff and scheduler.

- Strong nursing follows up and documentation skills

- Willingness to perform a variety of tasks which may not be conducive to strict nursing care.

- Demonstrates interest in continuing education, not necessarily formal classes.

- Perform any other duties or responsibilities that may become necessary or as directed by the supervisor.

Work Area and Working Conditions : Entire Health Center facility

Professional Qualifications :

- Graduate of accredited School of Nursing

- Current registration or license in the State of Connecticut

- CPR certified.

- Thorough knowledge of infection control policies and procedures and universal practices.

- Must be a team player with a positive attitude.

- Must have knowledge of geriatric, home care nursing with the desire and compassion to practice safe and effective care.

- Must be willing to attend in-services and perform a variety of simple tasks, which may not be conducive to strict nursing care.

- Perform any other duties or responsibilities that may become necessary or as directed by the director of Assisted Living Services.

Physical Requirements :

- Good physical and mental health.

- Must be able to lift 50-75 lbs., bend, sit stand and walk intermittently throughout the shift.

- Must be able to read, write, and speak in English in a legible and understandable manner.

- Must be able to cope with the mental and emotional stress of the position.

Equipment to be used :

Computer Printer all nursing and medical equipment

Key Accountabilities:

- Demonstrate the knowledge to be pro-active and preventative in residents care.

- Adhere to follow up needs with MD, Pharmacy, residents and family.

Demonstrates the ability to contribute to resident care conferences and develop and implement care plans.
