The Licensed Practical Nurse (LPN) provides direct patient care under the direction of a Registered Nurse (RN). This role supports patient assessments, care coordination, patient education, and communication with the healthcare team to ensure quality, compassionate care.

Position Details:

Key Responsibilities

- Assist with patient admissions, discharges, and ongoing care.
- Perform patient assessments and monitor changes in condition.
- Communicate patient updates and concerns to the RN and care team.
- Participate in bedside handoffs and patient safety initiatives.
- Provide patient and family education and support.
- Document care accurately in the electronic medical record.
- Collaborate with the interdisciplinary team to deliver high-quality care.
- Delegate and direct support staff activities as appropriate.
- Participate in unit meetings, huddles, and process improvement activities.
- Perform additional duties as assigned.

Qualifications

Required

- Graduate of an accredited Practical Nursing program.
- Current LPN license in the state of practice.
- Ability to meet all State Board of Nursing requirements.

Preferred

- BLS certification (required within 30 days of hire if not already obtained).
- ACLS, PALS, PEARS, S.T.A.B.L.E., or other unit-specific certifications as applicable.
- LPN-IV certification (or ability to obtain within required timeframe, if applicable).
