Position Summary
The Behavioral Health (BH) Veteran Coordinator provides support to CrescentCare Certified Community Behavioral Health Center (CCBHC) clients who have military experience. The overarching goal of the position is to utilize lived experience as a person with military experience to translate and explain services, treatment, and the recovery process to CCBHC clients with military experience as well as to provide navigation support to clients in enrolling in VHA benefits and accessing services both at the VHA as well as CrescentCare.
Key Duties and Responsibilities
- Utilize “lived experience” as an individual with miliary experience to translate and explain services, treatment, and the recovery process.
- Connect with each CrescentCare CCBHC client who has reported they have military experience to offer themselves as a peer resource with a focus on assisting with connection to VHA benefits to enhance access to physical and mental health services and supports.
- Connect with each CCBHC client affirming current or former military service and utilize “lived experience” to translate and explain services, treatment, and the recovery process.
- For CCBHC clients affirming current or former military service, assist in connecting the client to VHA benefits enrollment as follows:
- Persons affirming former military service (veterans) are offered assistance to enroll in VHA for the delivery of health and behavioral health services. Veterans who decline or are ineligible for VHA services will be served by the CCBHC. Assistance will be offered with the following:
1. Contact the military treatment facility regarding referrals outside the military treatment facility.
2. For members who reside more than 50 miles (or one hour’s drive time) from a military hospital or military clinic provide assistance enrolling in TRICARE PRIME Remote and use the network Primary Care Manager to refer the client to specialists for care he or she cannot provide and works with the regional managed care support contractor for referrals/authorizations.
3. For members of the Selected Reserves, assist with enrollment in TRICARE Reserve Select and assist with scheduling an appointment with any TRICARE authorized provider, network or non-network.
- Engage in active supervision and remain receptive to feedback and coaching from supervisor.
- Document all client encounters in the electronic medical record (EHR) within 3 business days.
- Attend treatment team meetings; consult with members of the treatment team.
- Attend all agency, department, programmatic, and supervisory meetings.
- All other duties as assigned.
Qualifications
Education
- High school diploma, General Educational Development (GED), or High school Equivalency Test (HiSET)
Work Experience
- One year of experience providing direct services in field similar/related to behavioral health and/or substance use disorders treatment/services.
Other (licensure, certifications)
- 21 years of age or older
- Must have military experience
ESSENTIAL PHYSICAL REQUIREMENTS
- Extended periods of sitting required.
- Work is primarily office based.
- Some travel may be required.
