Job Overview:
As a Licensed Practical Nurse with us, you'll be responsible for promoting the health and well-being of each resident by providing accurate nursing assessments, giving appropriate input to the development of care plans, and educating staff on the appropriate and proper care of each resident. You will also help to develop systems, procedures, and training programs that support governing state and federal regulations.
Compensation Range: $40.00 to $45.00 per hour (based on experience)
Shifts: Thursday-Monday, 3 p.m.–11 p.m. or Tuesday-Saturday, 8 hour shift. Shift days are flexible, but weekend availability is required.

Job Responsibilities (not limited to):
- Develop and support a system that allows for a constant flow of communication to the Director of Nursing regarding resident, family, or employee concerns, a decline in resident condition, employee injuries, budget disparities, or any matters that potentially impact operations
- Review and perform evaluations of resident health and functional care needs before moving in to ensure that the community can meet the residents needs and that regulatory requirements have been met
- Review all resident information and complete an assessment of the resident before move-in, when changes in the resident's condition occur, and as required by company policy and state regulation
- Assume on-call responsibilities on a rotational basis and assign on-call responsibilities to other qualified management
- Develop knowledge of community policies, procedures, and state/federal regulations pertaining to resident care
- Perform selected clinical tasks that reflect an unusual or unique condition in a resident or that cannot be delegated, i.e. IM injection
- Monitor the status of residents health condition and initiate actions in response to changing needs
- Assist in coordinating resident transfers, to or from a hospital or another health facility

Qualifications
- 18 years ofrolder
- Able to pass an Oregon background check
- Minimum 1 year LPN experience in related field/setting
- Possess and maintain strong knowledge of Oregon Board of Nursing requirements, especially as they pertain to Nursing Delegation
- Possess and maintain a strong ability to teach and train on all components of Nurse Delegation
- Possess a thorough knowledge of Med Room components and Medication Administration
- Must possess an active Oregon-endorsed LPN license
- Reliable Transportation
