About this role
Responsible for assisting activities of adult day care programs and services. Collaborates with staff and volunteers in identifying the health, education, and psycho-social needs of individuals in the community and develops programs, projects and activities based on the assessed needs. Assures that all patient needs are met in terms of personal care and all other activities.
Principal Accountabilities:
1. Creates social and educational activities to keep patients engaged and functioning, and manages the patient's behavior during these activities.
2. Tailors the day care activities to meet member specific needs and works to ensure the optimal quality of the activities.
3. Works in conjunction with social workers as they develop the programs and activities deemed appropriate.
4. Works to secure the physical spaces for the activities and validates the safety standards of each activity.
5. Collects feedback on the activities conducted.
6. Performs other related duties as assigned.
Required:
1. High School Diploma or equivalent.
Preferred:
1. 1 year of experience in assisted living, skilled nursing, or Adult Day Care experience or related field.
