The Case Management Assistant functions as a member of a
multidisciplinary health care team with guidance of the nurse case
managers and social workers, assisting in facilitating appropriate
patient placement; utilizes hospital and community resources for
continuity of patient care; collaborates with medical staff, HIM and
other hospital departments and staff. In addition to performing the
essential functions listed below, may also be assigned other duties as
required.
Reports to: Director of Case Management
1. High School graduate or equivalent required
2. Business college or Associate Degree desired
3. Previous healthcare experience preferred
4. Ability to read, write and comprehend medical terminology

Essential Job Responsibilities
1. Performs systematic ongoing assessment of Nurse Case
Manager and Social Work needs with focus on transitioning
patients safely and expeditiously out of the inpatient setting
2. Audits assigned processes and aggregates the data for
leadership review
3. Assists with DME inventory management
4. Updates white board as needed
5. Assists with Homeless supply management
6. Maintains daily discharge and transportation logs
7. Assists with employee timekeeping and scheduling
8. Takes minutes during Department Staff Meetings
9. Manages incoming and outgoing department mail
10. Faxes referrals to post-acute providers and follows up as
necessary
11. Manages department SharePoint resource space

ensures that direct and indirect services are provided for the
department
2. Functions to establish and review priorities of the departments
needs in an organized manner based on available resources of
time and personnel
