Nurse Care Manager & Patient Advocate (RN)

You will carry a small, intimate private-pay caseload — deliberately kept that way — so you actually know your clients: their history, their family dynamics, what a good day looks like for them. You will build your own schedule around your visits and exercise real clinical judgment without a charge nurse, a floor assignment, or a twelve-hour shift.

- $100,000–$115,000 base , plus a monthly performance bonus earned on billable hours and paid every month, never deferred to year-end. First-year total earnings run $106,000 to $130,000+.

About the role

Unicity Care Management supports individuals and families through the healthcare decisions that matter most — a new diagnosis, a complicated hospital discharge, a chronic condition that has outgrown what one family can coordinate on its own. Our clients range widely in age and circumstance. What they share is a moment where the healthcare system has become more than they can navigate alone.

As a Nurse Care Manager and Patient Advocate, you are the person your clients and their families call first.

You will:

- Conduct comprehensive in-home assessments covering clinical, functional, cognitive, psychosocial, and environmental needs
- Develop and maintain individualized care plans, revising them as circumstances change
- Coordinate physicians, home care, specialists, and community resources into a single coherent plan
- Manage transitions of care between hospital, rehabilitation, and home, closing the gaps where clients are most often lost
- Advocate for clients and families at appointments, care conferences, and points of decision
- Serve as the primary point of contact for the clients on your caseload, including responding to urgent needs that arise outside standard business hours
- This is a field-based role requiring regular travel and is not an in-office position. Provides high-touch, concierge-level nursing support to clients, including accompanying clients to medical appointments and other care-related visits as needed.

Requirements

What we are looking for

- Active Registered Nurse (RN) license in New Jersey
- A genuine interest in working alongside families, not only patients
- Sound clinical judgment in community settings, where the resources of a hospital are not at hand
- Strong written and verbal communication; you will be the clearest voice in the room for families under stress
- Organizational discipline to manage an independent caseload and maintain accurate documentation
- A collaborative approach to working with physicians, providers, and community partners
- Valid driver's license and reliable transportation for field visits
