Job Responsibilities:
- Conduct in-home client assessments and develop individualized plans of care
- Serve as a clinical liaison between caregivers, physicians, families, and the office team
- Train, guide, and support caregivers on care plans and safe practices
- Monitor client progress and revise care plans as needed
- Ensure compliance with state home care regulations and agency policies

Qualifications:
- Current RN license in the state of Georgia (Required)
- BLS Certification (Required)
- 2 years of RN experience in home health, geriatrics, or related field (Preferred)
- Excellent clinical and communication skills
- Reliable transportation, valid driver’s license, and auto insurance
- Clean background and driving record
