Position Overview

Under the direction of the Manager of Health Services, the incumbent supports the integrated clinical operations of the HOCS team and Access Downtown Social Workers.

HOCS service delivery supports complex community service integration for individuals experiencing houselessness or risk of losing housing. HOCS team support is provided primarily to other agencies, support providers, and system positions to better support individuals in this population through consultation and sometimes through direct intervention.

The team works in collaboration with WRHA programs, hospital sites, and other health and social services, and various community partners to provide care that is person-centered, trauma-informed and harm reduction focused.

Under the general supervision of the Manager of Health Services and as a member of a multidisciplinary team, the Clinical Team Coordinator provides support in facilitating quality client care. The Clinical Team Coordinator, in conjunction with the Manager of Health Services, is accountable for the effective management of the clinical, human, material and financial resources of the HOCS team health care initiatives.

Experience

- Four years related community experience with minimum of two years clinical coordination experience.

- Experience working in an integrated health care setting is essential.

- Experience working with vulnerable populations and people who are unhoused is essential.

Education (Degree/Diploma/Certificate)

- An undergraduate degree in health or social services is required.

SPECIAL TRAINING:

- Training or certification in a community health, primary care or acute care setting essential.

Certification/Licensure/Registration

- Responsible for maintaining and providing proof of active registration with the appropriate registering body.

- Valid Driver’s License and access to a vehicle required.

- Skilled in the practice of change management.

- Sound knowledge of and understanding of clinical evidence-based practice.

- An understanding and sensitivity for clients who are at risk.

- Knowledgeable in the fundamentals of Service Excellence, Customer Service and Cultural Safety.

- General knowledge of regulations and scope of practice documents that guide nursing and other related disciplines.

- Demonstrated ability to critically think, analyze, conceptualize and apply relevant knowledge of practice.

- Ability to work under minimal direction.

- Ability to facilitate professional continuing education requirements of staff.

- Knowledge of budgets and financial reporting systems.

- Embraces on-going learning and self-development.

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment.
