Summary
Within the context of a client and family centred model of care and, in accordance with the Mission, Vision and Values, and strategic directions of Providence Health Care, the person promotes a safe, respectful, and civil working environment for patients, residents, families, visitors and staff.Reporting to the Patient Care Manager or Resident Care Manager and working in collaboration with the interdisciplinary household team, the Household Licensed Practical Nurse (LPN) is responsible for providing nursing care to an assigned group of patients/residents according to established policies and procedures and the British Columbia College of Nurses and Midwives Standards for Licensed Practical Nurses. The Household LPN functions as an independent clinician, while recognizing situations such as changes in stability or predictability, or care that falls outside the scope or competence of the LPN, requiring consultation or referral to another health care team member including a Registered Nurse (RN) or Registered Psychiatric Nurse (RPN). The Household LPN assists residents in meeting their physical, social, spiritual, and psychological needs, consistent with the mission, vision and values of Providence Health Care (PHC) and established protocols. Participates in developing care plans, conducting assessments, implementing, and evaluating interventions, documenting information, and acting as an advocate for residents. The Household LPN is a member of the household team and works with the team to ensure that the abilities, preferences and routines of each resident are considered in decision making, and determining the order of the day. The Household LPN collaborates with other members of the household care team to share tasks and duties as appropriate to ensure the functioning of the household. Participates in a variety of meetings including care conferences and assists with orientation of new staff and students as required. Responds and reports incidents according to established procedures and performs clerical duties, as needed.

Minimum Nurse-to-Patient Ratios (mNPR) set the minimum number of nurses required to safely care for patients on a given unit. What You Bring
Education, Training and Experience
- Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience.
- Registration with the BC College of Nurses and Midwives (BCCNM) as a practicing LPN registrant.
- Current basic cardiac life support certification.
Skills and Abilities
- Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service.
- Demonstrated knowledge and competencies of a Licensed Practical Nurse as currently required by BCCNM.
- Demonstrated ability to communicate effectively, both verbally and in writing, with residents, families, co-workers, members of the multi-disciplinary care team and the public.
- Ability to deal effectively with others.
- Ability to organize work and implement a plan of care.
- Demonstrated ability to affectively model behaviours associated with the Home for Us model of care and mentor staff in the same.

Your Day to Day
- Provide safe and respectful nursing care, adhering to established policies, individualized care plans, and priorities
- Conduct thorough assessments, monitor signs/symptoms, and communicate with the RN/RPN as needed
- Prepare patients for procedures, transfers, and admissions, coordinating with the interdisciplinary team
- Administer medications per protocol, monitor effects, and maintain communication with the team
- Utilize evidence-based resources to stay updated on nursing practices
- Respond to incidents such as codes, injuries, spills, and alarms, following procedures
- Assist with staff orientation, demonstrating procedures and providing support
- Address patient/resident and family learning needs, provide educational materials, and make referrals
- Attend care planning meetings, provide input, and advocate for patients/residents
- Collaborate with the team to develop and evaluate individualized care plans
- Assist with discharge planning based on patient status and external supports
- Operate and clean equipment, report malfunctions, and handle hazardous materials. Perform clerical duties such as answering phones, faxing, and maintaining records
