Palliative Care Nurse Manager

Position Overview :

The Palliative Care Nurse Manager is responsible for developing and advancing the quality of nursing practice for the patients-residents through specialized direct patient care, leadership, coaching, collaboration, consultation, education and research. The Palliative Care Nurse Manager works closely with the Chief Nursing Officer and the Clinical Leadership Team.

Essential Functions :

- Works collaboratively with the facility leadership and staff to develop, implement and maintain a palliative care program.

- Educates physicians, staff, patients/residents and families on palliative

- Works collaboratively with Palliative Care Medical Director to deliver and expand palliative care services as appropriate.

- Develops, reviews and revises protocols, policies and procedures, order sets and processes specific to targeted population care needs.

- Collaborates with organizations to maintain and update professional standards of care, and participates in ongoing palliative care continuing education.

- Maintains primary responsibility for consult data tracking.

- Audits compliance with core metrics of programs developed to improve quality care

- In collaboration with Palliative Care Medical Director, routinely reports on program outcomes and data, along with recommendations for improvement to appropriate organization departments/leaders.

- Serves on committees, project groups and task forces as necessary to achieve facility’s goals and objectives.

- Interfaces with other departments to support practice issues specific to geriatric and palliative patients and staff who care for them.

- Provide clinical support (assessment, collaboration, education, interventions) as needed to all staff, for matters related to palliative care and palliative care patients.

- Manages panel of approximately 50-75 residents, determining follow up needs and triaging new consult requests.

- Comply with billing and documentation requirements with facility leadership.

Qualifications:

- Valid California RN License

- BSN required

- Minimum of 2 years RN clinical experience preferred

- MSN in Adult Gerontology preferred

- Palliative care experience preferred

Physical Demands:

- Ability to lift, push, pull a minimum of twenty-five (25)

- Ability to stand for extended periods of time

- Must be able to assist in evacuation of residents during emergency

- Ability to bend, stoop, stretch, twist, sit, and reach

- Fine motor skills

- Visual acuity
