We are seeking a Planetree Program Manager to help lead that work.

This is a highly visible, relationship-driven role for someone who is passionate about person-centered care, culture, human connection, and meaningful organizational change. You will serve as a champion for the principles of Planetree across Logan Health—helping leaders, caregivers, providers, patients, families, and communities translate those principles into everyday experiences.

What You'll Do

As the Planetree Program Manager , you will be a visible champion for person-centered care and help bring Logan Health's Planetree philosophy to life throughout the organization.

You will:

- Champion person-centered care by helping leaders and caregivers understand what it looks like in action and how it can shape everyday interactions and experiences.
- Build relationships across the organization, partnering with leaders, providers, caregivers, patients, families, and community partners to advance person-centered practices.
- Lead and coordinate Planetree initiatives that strengthen the experience of care and create meaningful connections between people.
- Develop and facilitate education and engagement opportunities that inspire caregivers and leaders to incorporate person-centered practices into their work.
- Help translate ideas into action, supporting departments as they implement person-centered practices and helping identify and remove barriers to success.
- Listen to the people we serve and the people who provide care, gathering feedback, stories, and insights that can inform meaningful improvements.
- Use data, feedback, and outcomes to evaluate initiatives, identify opportunities, and demonstrate the impact of person-centered care.
- Collaborate with Marketing, Foundation, Quality, Patient Experience, Human Resources, and other teams to amplify the work and celebrate the people making a difference.
- Tell the stories that matter, helping share examples of compassion, connection, innovation, and exceptional care throughout Logan Health.
- Represent Logan Health and its commitment to person-centered care through presentations, events, community engagement, and organizational initiatives.
- Maintain relationships with Planetree and other national organizations, bringing emerging ideas, practices, and resources back to Logan Health.
- Help advance Logan Health's journey toward excellence in person-centered care, continually looking for ways to make the experience better for patients, families, caregivers, and providers.

What We're Looking For

Required Qualifications

- Bachelor's degree in a related field or an equivalent combination of education and experience.
- Minimum of five (5) years of healthcare experience.
- Strong organizational, project management, and critical-thinking skills.
- Excellent written, verbal, presentation, and interpersonal communication skills.
- Demonstrated ability to build collaborative relationships across all levels of an organization.
- Ability to manage multiple initiatives, priorities, and stakeholders while maintaining follow-through.
- Ability to handle sensitive information with professionalism and confidentiality.
- Proficiency with Microsoft Office and ability to quickly learn new technology.

Preferred Qualifications

- Experience with Planetree or another person-centered care framework.
- Experience in patient experience, employee experience, organizational development, quality improvement, education, culture, or community engagement.
- Experience facilitating groups, leading organizational initiatives, or developing educational programs.
- Experience using qualitative and quantitative feedback to identify opportunities and measure impact.

Day Shift - 8 Hours (United States of America)

Schedule:

Logan Health operates 24 hours per day, seven days per week.  Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
