Are you passionate about supporting patients and families through life’s most vulnerable moments? Cabrillo Hospice is seeking a compassionate and dedicated Social Worker to join our interdisciplinary hospice team and make a meaningful difference in the lives of patients and caregivers.

What You’ll Do
- Conduct Comprehensive Psychosocial Assessments Assess the emotional, social, and environmental needs of patients and families, including caregiver stress, family dynamics, coping abilities, and safety concerns.
- Provide Emotional Support & Counseling Offer compassionate counseling to patients and families navigating serious illness, grief, and end-of-life transitions.
- Collaborate with the Interdisciplinary Team Work closely with nurses, physicians, chaplains, and other care team members to develop and support individualized plans of care.
- Connect Families to Community Resources Serve as a liaison to community agencies and support services to ensure patients and families have access to appropriate resources.
- Bereavement Support Participate in and support hospice bereavement services, providing follow-up and resources to families after the loss of a loved one.
- Ensure Compliance & Quality Care Maintain accurate documentation and ensure services align with hospice regulations, policies, and standards of care.
What We’re Looking For
- Master’s Degree in Social Work (MSW)
- Hospice or healthcare experience preferred (geriatrics or end-of-life care a plus)
- Strong interpersonal and communication skills
- Compassionate, patient-centered approach
- Ability to work independently while collaborating within a multidisciplinary team
- Cultural sensitivity and respect for diverse backgrounds and belief systems
