Scheduling Coordinator (Temp)

The Scheduler is responsible for managing and coordinating the schedules of home health professionals to ensure efficient and effective delivery of services to patients. This role requires exceptional organizational skills, the ability to prioritize tasks, and excellent communication skills to effectively liaise with healthcare professionals, patients, and their families. The Scheduling Coordinator will be responsible for scheduling patient visits, coordinating Home Health and Hospice referals, and maintaining accurate and up-to-date patient records.

This is a TEMPORARY position.

In this role, a Scheduling Coordinator will:

Coordinate and schedule patient visits with appropriate nursing staff based on patient needs, availability, and organizational protocols.

Ensure that all patient visits are scheduled within established timelines and according to the care plan.

Communicate with patients, nursing staff, and healthcare professionals to ensure that appointments are scheduled, rescheduled, or canceled, as necessary.

Manage and prioritize scheduling requests from patients, nursing staff, students, and Managers to ensure that all requests are handled promptly and efficiently.

Continuously monitor and adjust schedules as needed to ensure optimal utilization of resources and timely delivery of services.

Provide regular reports and updates to management regarding scheduling and patient care activities.

All other duties as assigned.

The ideal candidate will have:

- High School Diploma, AA or AS preferred.
- 3 years of Home Health and/or Hospice scheduling experience preferred.
- Ability to manage multiple tasks and priorities simultaneously.
- Strong organizational skills and attention to detail.
- Excellent communication and interpersonal skills.
- Ability to work effectively under pressure and in a fast-paced environment.
- Knowledge of medical terminology and patient care protocols.
- Proficiency in computer software applications such as Microsoft Office and Netsmart (preferred program).
- Ability to maintain confidentiality and comply with HIPAA regulations.

Compensation Pay Range:
$22.00 - $25.00 per hour

Reasonable Pay Estimate
A reasonable estimate of the pay range for this position is $22.00 - $25.00 per hour. There are numerous factors taken into consideration in determining the actual offered rate of pay, including but not limited to: job-related qualifications, experience, skills, education, geographic location, and consideration of internal and external equity.
