Sun River Health is currently seeking a Full-time Hepatitis C Linkage to Care Specialist to join our team located in Poughkeepsie, NY.
The Hepatitis C Linkage to Care Specialist will work with the Director of Nutrition and Operations to provide education and outreach for patients that are HCV positive
***$1,500 Sign on Bonus***
Responsibilities:
- Facilitates enrollment in HCV Benefits and Entitlement programs
- Maintains data and case records as required and prepares necessary reports
- Education among community agencies and internal staff members about the importance of Hepatitis C testing and treatment
- Educate providers and other staff about HCV testing, benefits of diagnosis and importance of linking to care
- Works cooperatively and collaboratively with entire HCV case management, Medical and Behavioral Health Provider teams
- Provide outreach to testing centers, education those newly diagnosed with Hepatitis C and referral to SRH, including tracking of all services
- Focuses on reducing barriers to ensure timely access to care and treatment.
- Develops linkages and internally refers patients for additional medical services, including treatment
- Work with Community Based Organizations (CBOs) to set up HCV screening services, provide community education and outreach and act as a liaison with CBOs to link HCV positive patients to care
- Participates in case conferences as well as other agency or community-based meetings or trainings

Pay: $22.00 - 25.47/hour
