JOB TITLE: Clinical Team Coordinator

DEPARTMENT : Clinical Services

POSITION SUMMARY

The Clinical Team Coordinator supports daily clinical operations, staff coordination, and workflow management to ensure the delivery of high ‑ quality hospice care for patients and families. This role reinforces regulatory compliance, strengthens interdisciplinary collaboration, and promotes a culture of professionalism, accountability, and continuous improvement. Working closely with the Team Leaders, the Clinical Team Coordinator provides operational oversight, assists with staff development, and ensures timely follow ‑ through on clinical and administrative tasks. The position serves as a reliable point of support for field staff, facilitates communication across departments, and helps maintain smooth, consistent functioning of the clinical team.

WORKING RELATIONSHIPS

REPORTS TO : Director of Clinical Services

SUPERVISES : None

INTERPERSONAL RELATIONSHIPS : All comments, actions and behaviors have a direct effect on the publics perception of Hospice Austin. Interactions with patients, family members, physicians, referral sources, visitors, co-workers, etc. must be in a manner that is courteous, respectful, cooperative and professional. This behavior should promote an atmosphere of teamwork and positive relations.

ESSENTIAL FUNCTIONS

1. Clinical Oversight & Patient Care

- Morning Meeting Support: Complete morning reporting in a timely manner. Assist with census review, staffing coverage checks, safety concerns, and overnight updates. Step in to lead morning meeting when Team Lead is unavailable.

- Task List Coordination: Manage uncovered visits, collaborate with Team Leads and field staff to ensure timely assignment and follow ‑ through. Email Task List to field staff and Reception in a timely manner and update throughout the day as needed.

- Support biweekly Interdisciplinary Group (IDG) meetings: Prepare materials, track POC updates, SMART goals, and eligibility items; support Team Lead during biweekly IDG and step in to lead when Team Lead is unavailable.

- Transfer Coordination: Assist with arranging transfers to HACH/respite, ensuring orders, communication, and transport steps are completed.

- Level of Care Monitoring: Track hospital LOC, continuous care documentation, PAD status, and SIA compliance; escalate concerns and update records when necessary.

2. Compliance, Quality, & Risk Management

- Documentation Audits: Review timeliness/accuracy of assessments, POCs, HOPE time points, supervisory visits, and terminal status evaluations.

- Timesheet Review: Lock and approve EMR timesheets, working with field staff to correct as needed.

- Oversee Relatedness Documentation: Track outside appointment requests and ensure documentation is complete in EMR and through the TAP form process.

- Ensure proper execution and charting of OOH ‑ DNR forms and other critical paperwork.

- Maintain emergency preparedness awareness and reporting expectations.

3. HR, Payroll & Staff Support

- Hiring & Onboarding Assistance: Participate in interviews, onboarding tasks, and orientation scheduling and follow ‑ up.

- Education & Compliance Monitoring: Track Relias completion, support staff education, and help coordinate team ‑ building activities.

- Review and approve Paycom timecards, expenses, punch changes, and PTO in a timely manner.

- Opportunities for Improvement: Assist Team Leads with documentation auditing and incident investigations for EthicsPoint Opportunities for Improvement and Medication Variances.

- Coordinate with DME vendor on DME issues, lost equipment, and escalations.

4. Leadership Presence & Professional Standards

- Professional Communication: Model calm, proactive communication and reinforce professionalism across the team.

- Facility Relationship Support: Maintain presence in facilities, assist with relationship ‑ building, and support Team Lead during facility visits.

- Safety & Improvement Culture: Reinforce safety practices, accountability, and continuous improvement initiatives.

- Office Presence: Maintain consistent presence at Spicewood office when not in the field or facilities.

5. Conducts other duties as delegated by Team Lead or Director of Clinical Services.

Qualifications

- Graduation from an approved school of professional nursing as determined by the Board of Nurse Examiners.

- Unrestricted license as a Registered Nurse by the Texas Board of Nurse Examiners.

- Valid Texas drivers license, auto liability insurance and safe driving record.

- Knowledge of hospice federal and state regulations.

- Strong computer skills including knowledge of and experience with Microsoft Office products and experience with electronic medical records (EMR).

- CPR certification.

- Strong clinical, communication, and clinical skills.

- Ability to manage multiple priorities in a dynamic, fast-paced environment.

- Compassionate, patient-centered approach with a focus on holistic care.

- Ability to handle emotional and difficult situations with professionalism and empathy.

PREFERRED SKILLS:

- Minimum of 2 years of nursing experience, with at least 1 year in hospice care preferred.

- Experience in leadership or supervisory roles.
