About this role
The PRN Case Manager provides as-needed care coordination services to support patient throughput, discharge planning, and quality outcomes in a hospital setting. This role collaborates with the interdisciplinary healthcare team to assess patient needs, identify potential barriers to care, and facilitate appropriate discharge plans. The PRN Case Manager supports patient care activities based on variable scheduling, patient volume, and operational demand, while promoting efficient resource utilization and continuity of care. Work is performed under the supervision of departmental leadership and in accordance with established clinical and operational guidelines, while upholding the mission, vision, and values of MSRH. The PRN role requires flexibility to work variable shifts, including on-call coverage and call-back as needed.
- Works effectively as part of an interdisciplinary healthcare team.
- Assesses patient needs and identify barriers to discharge, care coordination, and resource utilization.
- Strong problem-solving and critical thinking skills to support timely and effective discharge planning.
- Advocates for patients and navigate complex healthcare systems.
- Collaborates and communicates effectively with patients, families, physicians, and healthcare team members.
- Provides empathetic, patient-centered support to diverse populations.
- Knowledge of medical terminology, hospital operations, and care coordination processes.
- Familiarity with community resources, support services, and post-acute care options.
- Manages multiple priorities and adapts to changing patient needs and operational demands typical of PRN assignments.
- Documents patient care accurately and maintains confidentiality in accordance with regulatory standards.
- Proficiency in Electronic Medical Record systems and standard computer applications.
- Demonstrated commitment to supporting the mission, vision, and values of MSRH.
EDUCATION:
Bachelor's in Nursing/Master's Social Work
