## Job Description Summary
The Colorado Works Case Manager position may be hired at either Level I or Level II, depending on a candidate’s existing knowledge, skills, and experience related to Colorado Works case management and the Department of Human Services.

Colorado Works Case Manager – Level I
The Level I is an entry-level, learning-focused role designed to build foundational knowledge, skills, and professional competencies in Colorado Works case management. Employees hired at this level receive structured training, close guidance, and ongoing coaching from their supervisor and experienced staff.
Assignments are intentionally limited and gradually increase as the employee demonstrates competency in core functions, understanding of program requirements, sound decision-making, and the ability to meet timelines and performance expectations with growing independence.
Employees hired at Level I may be reclassified to Level II once they have successfully completed required training, demonstrated the ability to perform all essential functions independently, and consistently meet performance outcomes. While the timeline for progression varies by individual, it is generally not expected within the first year.

Colorado Works Case Manager – Level II
The Level II is a journey-level role responsible for directing and coordinating services that prepare Colorado Works participants for employment and long-term stability. Employees at this level independently manage a full scope of case management functions, including assessing participant needs, developing plans, and collaborating with employers, partner agencies, and educational institutions to support the participant’s path toward self-sufficiency.
Although Level II employees operate with substantial independence, supervisors remain closely involved and available for consultation, guidance, and support.

Level I:
$26.56 - $34.53 hourly

Level II:
$27.62- $35.90 hourly

Reliable, predictable attendance within department business hours of 8 a.m. to 5 p.m. Monday through Friday. This position is required to be in-person with a possibility for limited telework.

Job Description

Program Support - 65%

- Use and maintain database systems such as Connecting Colorado and Colorado Benefits Management System (CBMS) to support case management, and documentation requirements.
- Engage Colorado Works (TANF) participants to identify appropriate services, resources, and activities that support employment and long-term stability.
- Coach participants in developing and achieving short-term and long-term goals that support progress toward self-sufficiency.
- Collaborate with participants to develop individualized plans that reflect interests, aptitudes, barriers, strengths, and career goals.
- Assign participants to appropriate job readiness and skill-building activities in accordance with program rules and assessment outcomes.
- Monitor progress, attendance, and engagement in assigned activities to ensure participant is meeting program guidelines, objectives, and applicable state and federal performance standards.
- Conduct re-engagement meetings when a participant is not following or meeting expectations within their individualized plan.
- Evaluate participant needs on an ongoing basis and adjust plans as new information or circumstances arise.
- Achieve individual program requirements such as engagement in core activities, meeting timeliness standards, enrollment in work-based learning activities, and supporting employment outcomes that contribute to work participation rate (WPR) goals.
- Regularly assess the effectiveness of individualized plans in helping participants achieve immediate, intermediate, and long-term outcomes, adjusting plans as necessary.

Collaboration - 20%

- Partner with public and private employers, and the Business Services Unit, to create and support work-based learning opportunities aligned with participant needs and employment goals.
- Collaborate with the Assessment and Learning Center staff to interpret assessment results and incorporate recommended learning activities into individualized plans.
- Maintain a professional working relationships throughout the Department of Human Services, other County departments, schools, employers, service providers and community-based organizations.
- Review requests for supportive services and authorize, or seek approval for, those services in accordance with program guidelines, policy, and regulations.
- Maintain current knowledge of community resources and partner agencies to ensure responsible and effective use of program funding.
- Involve community partners in planning, staffing, or support meetings when appropriate to enhance participant outcomes.
- Share pertinent information with partners in compliance with confidentiality requirements and applicable regulations.
- Follow up with community partners on agreed upon activities in a timely manner and professional manner.

Administrative - 15%

- Accurately establish and maintain electronic participant files in accordance with Department policy and applicable regulations, ensuring complete and timely documentation of employment plans, attendance and progress records, supportive services, case notes, and all other required documentation.
- Track and monitor participant activities by entering accurate, timely data into required systems and reviewing information regularly to ensure compliance, performance tracking, and audit readiness.
- Maintain detailed, accurate case notes that reflect participant contacts, decisions, actions taken, changes to the individualized plan, and any relevant information required for State or Federal reporting.
- Ensure documentation meets quality assurance standards and is sufficient for State review, County audits, and compliance with TANF regulations.
- Upload, organize, and maintain digital records in County systems to ensure accessibility, consistency, and secure handling of participant information.
- Protect participant confidentiality and adhere to all legal, regulatory, and County requirements regarding information security.

Required Qualifications

Required Education

- High School Diploma or equivalency.

Experience Qualifications – Level I

- 1 year of business or clerical experience in an office environment.
- Equivalent combination of education and experience may be considered in lieu of degree.

Experience Qualifications – Level II

- 3 years of business or clerical experience in an office environment which includes a high level of participant service and proven experience working with the public.
- Equivalent combination of education and experience may be considered in lieu of degree.

Preferred Experience

- Previous experience working in a Human Services environment.
- Previous case management experience.
- Previous experience working with Colorado Benefits Management System (CBMS).

Skills and Abilities

- Preferred ability to read and write Spanish in a proficient manner. Ability to interpret and translate between English and Spanish using professional language.
- Candidate must demonstrate proficiency with computer technology.
- Candidate must possess the ability to enter data into several different computer programs.
- Candidate must possess the ability to learn and apply new computer skills on a regular basis.
- Candidate must possess knowledge of computer programs such as Microsoft Windows, Word, Excel, PowerPoint, OneNote, Teams, and Outlook.
- Candidate must possess the ability to read and comprehend instructions, correspondence and department policies and procedures.
- Candidate must possess the ability to write correspondence.
- Candidate must possess the ability to effectively communicate in one-on-one and small group situations to participants and other employees of the organization.
- Candidate must possess the ability to apply math concepts and calculations in the work environment.
- Candidate must possess the ability to apply common sense to problem solve in a work environment.
- Candidate must possess the ability to work independently and handle high-stress situations, prioritizing competing tasks or responsibilities in a fast-paced, high volume, and ever-changing work environment.
- Candidate must possess the ability to adhere to strict confidentiality guidelines and appropriate release of information.
- Must possess ability to keyboard at 35 words per minute.
- Candidate must pass criminal background check prior to employment start date.

Licenses and Certifications

- Candidate must pass Department of Human Services background checks prior to employment start date. (Required)
- Driving is essential in this position.
- Candidate must have a valid Driver’s License and Liability Insurance. Candidate must provide own transportation and be willing to travel as needed to meet the essential functions of the position. (Upon Hire Required)
- Candidate must pass a Motor Vehicle Record (MVR) evaluation and if hired, will be subject to continuous monthly MVR monitoring and random drug screens throughout employment. (Required)
- Pre-employment substance screening is required for this position.
- This position is subject to a pre-employment drug screening. Employment is contingent upon successfully passing the screening in accordance with Weld County policy.

This position is non-exempt from the minimum wage and overtime requirements of the Fair Labor Standards Act and is therefore eligible for overtime pay.
