Home Health Services of Virginia is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to join our team in Henrico County, VA. As an LPN, you will play a critical role in providing high-quality, personalized care to our patient in a group home setting. This position offers a rewarding opportunity to make a positive impact on the lives of individuals who require nursing assistance, while working in a supportive and professional environment.
The Hours available are Monday-Friday 4pm-10pm!
Responsibilities
- Deliver skilled nursing care to patients in their homes according to the personalized care plan developed by healthcare providers.
- Monitor patients health by checking vital signs, observing symptoms, and reporting any changes or concerns to the supervising Registered Nurse or physician in a timely manner.
- Administer medications and treatments safely and correctly, ensuring adherence to established protocols and patient-specific instructions.
- Provide wound care, catheter care, and assist with mobility and personal hygiene to promote patient comfort and recovery.
- Maintain accurate and detailed patient records, documenting care provided, patient progress, and any relevant observations.
- Educate patients and their families about managing illnesses, medication schedules, and lifestyle adjustments to improve overall health outcomes.
- Collaborate closely with interdisciplinary healthcare teams to coordinate care plans and support patient needs effectively.
Requirements
- Current and valid Licensed Practical Nurse (LPN) license in the state of Virginia.
- Previous experience working as an LPN in home health care, long-term care, or a related clinical setting is preferred.
- Strong understanding of nursing practices, infection control, and patient safety protocols.
- Excellent communication and interpersonal skills to interact compassionately with patients, families, and healthcare team members.
- Ability to work independently, manage time effectively, and adapt to a variety of patient needs and environments.
- Reliable transportation to travel within Henrico County to visit patients homes.
- Basic proficiency with electronic health records (EHR) and documentation software is a plus.
