FlexStaff is seeking an extraordinary, dedicated LPN to join our client’s innovative Outpatient Clinical team. This isn't just a nursing role – it's a pivotal opportunity to lead, educate, and advocate, making a profound difference in the lives of our patients.

Responsibilities:

- Take the helm in training all clinical staff on our vital "Partnership for Health" and medication adherence interventions, ensuring seamless integration and best practices across the clinic. Coordinate booster training and champion continuous learning!
- Engage patients in meaningful, life-changing dialogue. You'll be their champion, providing essential education on prevention, risk reduction, sexual health, and substance use. Guide them through treatment adherence, supporting medical appointments and medication regimens, including antiretroviral therapy, with a truly patient-centered, holistic approach.
- Be the central figure in managing patient flow, coordinating crucial follow-up appointments, and acting as a vital liaison between patients and medical providers. Facilitate access to essential medical and social support services, ensuring no one falls through the cracks.
- Leverage your proficiency with Electronic Medical Records and patient databases to meticulously track patient data, monitor intervention fidelity, and report outcomes. Your analytical skills will ensure our programs are effective, evidence-based, and continuously improving.
- Lead daily huddles, proactively identify patients needing follow-up for chronic health maintenance, prevention, or other needs, and coordinate outreach. You'll foster a culture of safety, effective communication, and teamwork that resonates throughout our clinic.

Qualifications:

- Licensure: A current, active Licensed Practical Nurse (LPN) license in New York State.
- Specialized Certification: HIV Specialist certification is REQUIRED.
- Minimum of an Associate Degree or equivalent certification.
- At least 3 years of job-related nursing experience.
- A minimum of 1 year in an ambulatory care or specialty clinic setting is essential.
- 1-3 years of HIV-related experience is strongly preferred.
- Tech Savvy: Proven proficiency with computers, Microsoft Office products, and Electronic Medical Records (EMR).
- Outstanding interpersonal and communication skills that project a positive image.
- A natural ability to engage patients, prioritize workload, and manage a dynamic caseload.
- The capacity to work independently while also functioning as an active, collaborative team member.
- A strong orientation towards consumer satisfaction, high attention to detail, and demonstrated proficiency in reconciliation.
- Ability to create front-end infrastructure for clinics, such as scheduling.
- Bilingual Bonus: Fluency in English and Spanish is a significant advantage!
