JOB SUMMARY

The Behavioral Health Therapist is a member of the interdisciplinary team committed to focusing on the individual health care needs of adult patients within the Duke Signature Care concierge practice, a Primary Care Medical Home. The Behavioral Health Therapist collaborates with the patient, physicians, and other members of the health care team to provide assessments and formulate treatment plans using a holistic approach to care.

JOB DUTIES AND RESPONSIBILITIES

- Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation. Formulate, develop, and implement a comprehensive psychosocial treatment plan utilizing appropriate treatments and interventions to treat and manage mental health and/or substance abuse problems
- Document assessment, plan, interactions, and interventions in the electronic health record according to departmental, hospital and/or health system guidelines and standards. Maintain records in accordance with department, hospital, medical center and/or health system policies.
- Provide education to patients and loved ones around issues related to adaptation to the patient's diagnosis, illness, treatment and/or life situation; consultative services to health care team members within scope of care; teaching and training for staff and faculty.
- Maintain a working knowledge of health system resources to access and relevant medical/legal issues that impact patient care; liaison with community agencies and resources to further support patient care needs.
- Collaborate with members of the interdisciplinary team to identify and resolve issues and improve work flow processes. Attend and participate in huddles, staff, committee, and departmental meetings; identify and engage in continuous quality improvement initiatives.
- Adhere to licensure, health system, and regulatory guidelines and requirements; complete annual and required trainings within defined timelines; other projects and duties as directed by clinic leadership.

JOB ELIGIBILITY REQUIREMENTS

- Master's degree in human services/sciences from an accredited school. Doctorate degree in human services will be acknowledged
- Two years of outpatient psychotherapy experience as a fully licensed, autonomous clinician. Experience working as a licensed therapist in a medical setting and/or in a team-based community program is preferred.
- Current licensure as a licensed LCSW, LMFT, and/or LCMHC. LCAS is preferred as a secondary license.

JOB HOURS

The schedule for this position is day shifts, Monday-Friday, with no weekends or holidays.
