About the Role

Baylor Scott & White Health is seeking an experienced Registered Nurse (RN) Transfer Coordinator to join our Transfer Center in Temple, TX. This onsite nursing position is ideal for an experienced RN with a background in Critical Care, Emergency Department (ED), Intensive Care Unit (ICU), or Post-Anesthesia Care Unit (PACU) who is looking to apply their clinical knowledge in a centralized patient transfer and care coordination role.

As a Transfer Coordinator RN, you will facilitate patient transfers throughout the Baylor Scott & White Health system by coordinating communication between referring physicians, nurses, hospitals, patients, and internal departments. You will use your clinical expertise, critical thinking, and communication skills to help coordinate appropriate placement and support efficient patient movement across the health system.

Job Summary

The Transfer Coordinator RN supports patient movement throughout Baylor Scott & White Health by coordinating transfers and facilitating access to appropriate facilities, providers, and resources through the Transfer Center.

This role requires an experienced Registered Nurse who can apply strong clinical judgment and critical care knowledge in a fast-paced, centralized environment. The Transfer Coordinator RN works closely with physicians, nurses, hospitals, and other healthcare professionals to gather clinical information, coordinate patient placement, and support safe and efficient transfers.

The ideal candidate will have previous Critical Care RN experience, including experience in the ICU, Emergency Department (ED), or PACU, along with strong communication, organization, and problem-solving skills.

Essential Functions
Coordinate patient transfers across the Baylor Scott & White Health system by collaborating with physicians, nurses, hospitals, and other healthcare departments.
Review and gather accurate clinical information from referring hospitals, physicians, and healthcare professionals to support appropriate patient placement and transfer decisions.
Apply nursing knowledge, critical thinking, and clinical judgment when coordinating patient transfers and accessing appropriate healthcare resources.
Follow applicable laws, regulations, accreditation standards, clinical protocols, and Baylor Scott & White Health policies.
Provide exceptional customer service to referring physicians, healthcare organizations, patients, families, and internal departments.
Communicate clearly and promptly with healthcare professionals and other stakeholders throughout the transfer process.
Ensure transfer documentation is accurate, complete, clear, concise, and compliant with established standards.
Utilize available Baylor Scott & White Health resources to support patient placement and efficient hospital operations.
Build and maintain positive relationships with referring physicians and healthcare organizations through timely communication, follow-up, and issue resolution.
Support customer satisfaction and relationships that contribute to the continued growth and service of Baylor Scott & White Health.
Oversee and assist the Transfer Center Room Control Coordinator as needed.
Manage multiple transfer requests and priorities in a fast-paced environment while maintaining attention to detail and patient safety.

Key Success Factors
Broad knowledge of nursing practices, patient care standards, and clinical procedures.
Previous Critical Care RN experience in ED, ICU, or PACU required.
Knowledge of applicable healthcare laws, regulations, accreditation requirements, and professional standards.
Understanding of hospital and department policies and procedures.
Strong knowledge of medical terminology, disease processes, risk reduction, and patient care practices.
Knowledge of commonly used medications, including indications, dosages, effects, and potential side effects.
Excellent verbal and written communication skills.
Strong critical thinking, clinical judgment, organization, and problem-solving skills.
Ability to effectively coordinate multiple priorities in a fast-paced healthcare environment.
Proficiency with Microsoft Office, email, scheduling, and related computer systems.

- Associate's degree in a relevant field is required.
