POSITION SUMMARY:

The Referral Coordinator Team Leader is responsible for assisting the Patient Center Manger with the management of referrals of patients to outside agencies, and the appropriate follow up of those referrals per established policies and procedures.  The Referral Coordinator Team Leader reports to the Referral Supervisor.

SPECIFIC DUTIES:

- Promotes the mission, vision, and values of the organization.

- Assists with interviews for referral coordinator positions with the Patient Center Manager.

- Schedules, monitors, and assigns specific duties to referral coordinators.

- In conjunction with the Patient Center Manager, completes monthly referral reports.

- Responsible for updating, educating, and auditing staff to all changes in policies and procedures, coordinated with Patient Center Manager.

- Assists Patient Center Manager with evaluations and disciplinary procedures.

- Coordinates and participates in orientation and training of referral coordinators.

- Assists referral coordinators with referral problems and addresses referral questions and concerns.

- Reviews timecards independently or in conjunction with Patient Center Manager.

- Coordinates the referral of patients and appropriate follow-up of referrals per established policies and procedures.

- Completion of follow-up on all referrals including:

Insurance authorizations

- Transmission of a “summary of care” record and other medical information to specialist

- Appointment scheduling or rescheduling

- Patient notification of appointments and requirements

- Receipt of consult notes and attaching them to the medical record

- Referral tracking activities to include all of the above elements

- Answers the telephone and provides information with respect and courtesy while also being effective and efficient.  Communication methods may require interpretation and translation.

- Ongoing knowledge, documentation and up-date of appropriate referral resources required forms and key contact persons.

- Continued resource for outlying sites regarding referral program and resource contacts.

- May be involved in other programs as assigned.

- Performs other duties as assigned.

Qualifications

MINIMUM REQUIREMENTS:

- Completion of high school or possession of GED

- Two (2) years of experience as a referral coordinator or similar type duties working with the referral of patients for treatment and diagnostic studies, or, Three (3) years of full-time experience as a medical receptionist or medical assistant, 1 year of experience with referrals.

- Communicate effectively by using welcoming words, proper tone of voice, appropriate body language, eye contact and smiling with every interaction.

- Ability to provide excellent customer service that is reflective of a culture that values trust and respect.

KNOWLEDGE, SKILLS, AND ABILITIES:

- Knowledge of business office procedures

- Knowledge of ICD-10 and CPT codes

- Knowledge of all CMC program referral requirements

- Knowledge of Managed Medi-Cal and EDS Medi-Cal Tar Requirements

- Knowledge of all insurance pre-authorization requirements and documentation

- Knowledge of grammar, spelling, and punctuation to type patient information.

- Skill in operating computer, including population management/tracking software and photocopy and facsimile (FAX) machine.

- Skill in greeting patients and answering the telephone in a pleasant and helpful manner.

- Ability to communicate clearly.

- Ability to establish and maintain effective working relationships with patients, employees, and the public.

TYPICAL PHYSICAL DEMANDS:

Work may require sitting for long periods of time; also stooping, bending, and stretching for files and supplies.  Occasionally lifting files or paper weighing up to 35 pounds.  Requires manual dexterity sufficient to operate a keyboard, operate a calculator, telephone, copier, and other such office equipment as necessary.  Vision must be correctable to 20/40 and hearing must be in the normal range for telephone contacts.  It is necessary to view and type on computer screens for long periods and to work in an environment which can be very stressful.

TYPICAL WORKING CONDITIONS:

Work is performed in an office environment within a clinic setting. Involves frequent contact with staff and the public. Work may be stressful at times. Contact may involve dealing with upset people.
