GENERAL DESCRIPTION OF POSITION:
Responsible for the management of the daily departmental operations of the Pediatric/Adult Department at Brownsville and functions under the direction of a CNO of a given patient care area; the direct supervision of Medical Assistants (MAs), Care Managers (CMs), Certified Nurse Assistant, and may provide direct patient care daily. Performs functions to enhance quality, address prevention, increase compliance, improve patient satisfaction, maximize efficiency of providers/nurses, and meet Accountable Care Organization (ACO), Primary Care Medical Home (PCMH), and Meaningful Use indicators (MUIs). Serves as a liaison between the physicians and the clinical staff in patient care related issues. Implements and follows Su Clinica (SC) policies and procedures. Provides assistance and guidance to the clinical staff. May be required to work evenings and weekends as needed. Performs other duties as assigned. As a member of the clinical team, this position requires total support of the Patient Centered Medical Home (PCMH) guidelines, policies, and procedures. Performs duties to ensure the patient has a pleasant and satisfactory experience in the manner in which their health care is delivered, managed, and coordinated with both internal and external entities and in which the patient participates in decision making. Works with the multidisciplinary team in overseeing medication and nursing services, coordination of care and care management, policy and procedure management and development. This position may require to float to other departments within SC besides assigned primary location and/or to another SC location.
ESSENTIAL JOB FUNCTIONS: (with or without accommodations)

- Supervises clinical operations of a given patient care area
- Anticipates the needs of the Primary Care Provider and carries out orders
- Performs simple to moderately complex nursing procedures within the scope of practice of an LVN
- Obtains, interprets, records and reports vital signs (temperature, pulse, respiration, blood pressure, and pain scale), height, weight and other pertinent patient information to the clinician
- Under the direction of CNO may implement a plan of care by performing nursing interventions and assisting with procedures
- Performs various patient tests and procedures within the scope of practice of an LVN
- Administers medications and other therapeutic modalities within the scope of practice of an LVN
- Maintains clinical standards outlined the nursing clinical policies and procedures; identifies deficiencies and makes recommendations for change.
- Documents and maintains records of medical and nursing procedures
- Coordinates with clinicians and ancillary departments to plan daily work assignments according to teammates abilities, patient and clinician schedules, patient flow and procedures
- Ensures that all equipment is maintained
- May perform telephone screening
- Manages ordering and maintaining of all supplies
- Notifies pharmacies of new prescriptions and authorizations of refills as ordered by provider
- Participates in educational programs to enhance nursing knowledge
- Adheres to Standard Precautions at all times
- Collaborates with Clinic Manager with daily operations of department
- Ensures compliance with the PCMH Guidelines
- Evaluates staff member competencies
- Performs hiring, terminations, and evaluations
- Coordinates teammate coverage and ensure proper staffing/scheduling/care management rotations
- Assists with Quality Metrics, telephone follow up procedures
- Identifies and participates in quality improvement projects
- Uses, protects, and discloses our patients protected health information only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards
- Ensures monthly expiration logs for medications and sample medications (refrigerated and non-refrigerated) are maintained
- Ensures supplies are not expired
- Ensures yearly expiration logs for Preventative Maintenance are performed as needed
- Ensures abnormal test results are followed up on as directed by provider
- Ensures referrals are made as appropriate
- Ensures TVFC/ASN vaccine programs are compliant, and enrollment is up to date with Cameron County Public Health.
- Ensures immunization records and updating electronic medical record (EMR) are current
- Monitors overdue orders and follows approved process to address the orders
- Assists providers with medication reconciliation per guidelines
- Uses approved standing orders to perform, order tests, and update referrals
- Assists with entering referrals in electronic health records EMR for clinician to authorize
- Attends/participates in mandatory annual skills validation for self and staff and other clinical updates that may take place during evening hours and or weekends
- Attends all Nursing Supervisor meetings unless other arrangements made to be absent
- Assists clinician and licensed nursing teammates in responding to patient emergency situations as per Emergency Response Protocols
- Facilitates staff/clinician huddles
- Works in collaboration with a provider to prepare, monitor and discharge patients after a procedure
- Ensures exam and clean supply rooms are clean and stocked appropriately
- May cover for other LVN Supervisor in their absence (i.e. of paid time off, leave of absence)
- Assists with departments specific functions within the LVN scope of practice
- Verifies that the informed consents for procedures have been signed, dated and witnessed
- Prepares labels, collects and transports various specimens to appropriate destinations
- Cross trainings to work in other departments or other duties as assigned by supervisor /management
- Performs additional duties as assigned
Patient Relationship:
- Greets and welcomes the patient in a pleasant, respectful, culturally and linguistically appropriate manner
- Assures all patient questions are answered in a positive and cordial manner
- Assures the patient has a clear understanding of all communications
- Communicates patients problem/complaint to the provider
- Reinforces to the patients the importance of keeping appointments or communicating changes in their appointments
- Asks the required questions for a proper data collection taking into consideration the age of the patient
- Ensures that staff is properly trained to provide a clear explanation to patients of all communications
Management of Patient Care:
- Provides training to staff so that staff is able to prepare patients for examinations and treatments, take history and vital signs, review alerts, medications, initiate notes in the electronic medical record (EMR), and collect and process biological specimens
- Prepares examination and treatment rooms with necessary equipment and supplies
- Gives injections that are in their scope of practice
- Assists with refills, reviews all patient medications as per protocols
- Initiates notes for the nurse on call or others to alert information regarding a patient
- Assists the providers as requested
- May collect and process biological specimens for analysis and other lab tests
- Assists with scheduling tests or treatments as requested
- Supervises and conducts competency assessments of the staff
- Plans and prepares staffing schedules to accommodate the providers and patients
- Reports issues of operational concern to the CNO in a timely manner
- Supervises, performs chart audit, and team meetings to keep staff current on patient care changes, new processes and trends
Manages Patient Information:
- Takes and documents patients history and vital signs
- Performs pre-visit chart reviews for required health indicators, vaccines, labs, etc.
- Reviews chart under the direction of provider and obtains and places report results in charts
- Makes list of anticipated needs for patients (vaccines, labs, etc.)
- Assures the patient information and records is completed timely and is current
- Adheres to all HIPAA regulations in and outside of the Clinic
- Trains and enforces the importance of patient confidentiality and HIPAA regulations by the nursing support staff
- Communicates patient information as requested to enhance patient services
- May be required to supervise/participate in one or more Outreach Programs, supporting and collecting data for deliverables associated with grants, and working with the ACO, PCMH, and MUIs
Team Communication:
- Oversees daily operation of nursing unit
- Ensures providers productivity & schedule is reviewed & populated throughout the day
- Works on same day access
- Ensures employees/students are properly oriented & trained
- Communicates specific patient needs &/or population management information
- Interviews & recommends selection of nursing staff
- Conducts performance appraisals, employee counseling of staff
- Ensures staff is informed & abides by operational procedures/policies
- Serves as a liaison between provider, clinic departments & nursing staff
- Reports issues/concerns to CNO in timely manner
- Assists provider during patient examination
- Ability to interpret (explain the meaning of spoken words)and translate (to write) for the provider when necessary preferred
- Sets-up for procedures & assist the provider as needed
- Provides assistance & guidance to nursing staff
- Coordinates care with inter-disciplinary team and community partners

- Participates in committees, projects, and Performance Improvement initiatives
Supervisor:
- Leads and assist nursing team with duties above
- Attends clinic meetings and other staff meetings, communicate info to nursing team
- Prepares medical and drug histories for patient records
- Provides appropriate nursing input into medical treatment planning
- Prepares/manages equipment for dispensing
- Maintains regulatory standards and organizational systems
- Works with multi-disciplinary team in maintaining agency policies, procedures and philosophies
- Completes Incident Reports, troubleshoot resolutions with team
- Communicates to other staff concerns regarding specific patient issues
- Participates in weekend rotation as needed
- Partners with Human Resources to recruit, interview, and train nurses
- Directs care for patients, facilitating work flows, serving as a resource for staff, and providing team leadership
- Ensures the Medication Room of the clinic is in full compliance with all facility or clinic policies, Federal and State Laws and Regulations at all times
- Supervises daily clinical operations for staff, makes staff assignments and creates schedules, updates schedules for providers and support staff when changes are made
- Supervises and evaluates work activities of nursing, technical, clerical, service or maintenance staff for the work group, and other clinic staff, as directed
- Ensures that medical records for patients are in compliance with facility or clinic policies, Federal and State Laws and Regulations at all times, will review medical records to ensure compliance
- Oversees quality assurance activities and compliance to operational standards, participates in program development, development or implementation of operating policy and procedure, and promotes continuous improvement of workplace safety and environmental practices, as requested

Qualifications
KNOWLEDGE, SKILLS, AND ABILITIES: (use of equipment, job related knowledge, language, etc.)
- Knowledge of practice level procedures, techniques, and practice of patient care

MINIMUM QUALIFICATIONS/CERTIFICATES/LICENSES/REGISTRATIONS REQUIRED: HS Diploma or GED. Current licensure as an LVN in the State of Texas is required. Two (2) years of clinical experience in supervising personnel in a healthcare setting, preferred. Must be current with BLS/CPR certified. Experience in working with the public preferred. Must be computer literate. Bilingual (English/Spanish) a must. This position requires an initial N95 mask fit testing and annual test fitting thereafter unless there is a significant change during the year that requires retesting prior to the annual fit testing. Strong supervisory, organizational, and leadership skills. Ability to work with complex cases and vulnerable population .
SPECIAL INSTRUCTIONS : This position requires travel. Employee must provide transportation. If employee operates a personal motor vehicle in the performance of their official duties, the employee must possess a current valid Texas drivers license for the appropriate type of vehicle and Texas Liability insurance. This position requires that an employees driving record be verified with law enforcement to ensure compliance with SCs driving policy.
Due to the nature of SC Services, it may be necessary for employees to work extended hours or other variations of the usual shift to ensure adequate care to patients and to maintain service to the community.
