JOB SUMMARY
The Promotora serves as a trusted community advocate and representative of Via Care, helping connect individuals and families to healthcare services and resources. Through outreach, health education, and enrollment support, the Promotora builds relationships within the community, promotes awareness of Via Care's programs and services, and assists community members in accessing care. This role plays an important part in improving health awareness and connecting underserved communities to available healthcare and support services.
DUTIES AND RESPONSIBILITIES
- Provide culturally responsive health education to individuals and families on topics that support overall health and wellness.
- Conduct community outreach activities to promote Via Care's healthcare services and programs.
- Build and maintain positive relationships with community members, schools, local businesses, faith-based organizations, housing sites, and community-based organizations.
- Recruit participants for health education activities, enrollment events, and community programs.
- Represent Via Care at health fairs, community events, educational workshops, and outreach activities.
- Provide culturally responsive health education and information regarding available healthcare services, including primary care, dental, behavioral health, women's health, senior services, and urgent care.
- Assist community members in accessing healthcare services, community resources, and preventive care programs.
- Support patient enrollment and registration efforts, including appointment scheduling and referrals as needed.
- Conduct outreach calls and follow-up activities to encourage participation in services and community programs.
- Distribute educational materials and promote awareness of Via Care programs and services.
- Maintain accurate records of outreach activities, contacts, referrals, and event participation.
- Collaborate with Outreach and Enrollment staff to identify opportunities for community engagement and growth.
- Participate in required trainings, staff meetings, and outreach events.
- Maintain confidentiality and comply with organizational policies and HIPAA requirements.
- Perform other duties as assigned.
Qualifications
QUALIFICATIONS
- High school diploma or GED.
- Bilingual in English and Spanish required
- Preferred: One (1) year of experience in community outreach, health education, enrollment services, customer service, public health, or a related field. Volunteer experience may be considered.
- Strong communication and interpersonal skills.
- Ability to build trust and establish relationships with diverse community members.
- Ability to work independently and as part of a team.
- Ability to travel throughout Via Care's service areas and attend occasional evening or weekend events.
- Commitment to serving diverse, underserved, and multicultural communities.
SPECIAL REQUIREMENTS
- Preferred to have a reliable automobile for use on the job (mileage to be reimbursed)
- Subject to a criminal background check prior to employment
- TB clearance, to be renewed every year. COVID Vaccine and boosters as needed.
