Intake Manager LPN or RN Home Health and Hospice

Kline Galland Benaroya Services is seeking an experienced Home Health & Hospice Intake Manager, LPN or RN to lead our intake, referral, eligibility, and scheduling functions for our growing Home Health and Hospice programs.

Job summary: Oversees the end-to-end referral/intake, eligibility verification, and scheduling process to ensure patients transition into care smoothly, quickly, and compliantly.

RN Registered Nurse pay scale: $93,600 to $104,000 a year
LPN Licensed Practical Nurse pay scale: $87,360 to $99,840 a year

Core Responsibilities:

- Operations Oversight: Manage the daily workflow of the intake and scheduling teams, ensuring timely processing of incoming referrals.

- Eligibility & Compliance: Review patient records for home health and hospice appropriateness and verify insurance authorization under Medicare, Medicaid, and private payer guidelines.

- Serves as primary referral screener and responder to all incoming referrals into AIDA, Wellsky, fax and email.

- Clinical Expertise: Assures clinical appropriateness of all home health and hospice admissions

- Coordinates complex care transitions from hospital to home health or hospice service, including coordinating including wound care agreements, pain pumps or other specialized services.

- Screens referrals from hospice for clinical complexities and coordinates needed services such as pain pumps and or wound care contracts

- Stakeholder Collaboration: Partner with hospital discharge planners, physicians, and internal liaisons and clinical teams to clear barriers to admission.

- Builds and monitors community and customer perceptions of Kline Galland Hospice and Home Health Services as a high-quality provider of services.

- Works collaboratively with Home Health and Hospice Leadership team members to drive admissions and growth, while maintaining excellence in care and customer service.

- Team Leadership: Supervise, train, and mentor intake coordinators and support staff.

- Is part of the Benaroya Services leadership team, promoting process improvement and efficiencies and organizational performance improvement initiatives.

- Participates in the Administrator On Call rotation

- Performance Tracking: Monitor key performance indicators (KPIs) like referral conversion rates and time-to-admission to drive efficiency.

Qualifications

Qualifications & Requirements

- Experience: 2 to 5 years of healthcare experience, with a strong background in home health and hospice plus 1+ years in a supervisory role.

- License: LPN Licensed Practical Nurse or RN Registered Nurse

- Regulatory Knowledge: Deep understanding of Medicare Conditions of Participation, state Department of Health regulations, and insurance requirements.

- Technical Skills: Familiarity with Electronic Health Record (EHR) systems, MS Office Suite.
