Job Description
The Caregiver (CNA/CHHA) will report to Health & Wellness Director. The Caregiver is a Part-Time, non-exempt, hourly position.
Schedule:
Every Friday-Monday 7a-3p or 3p-11p
Salary Range: $21.00 - $22.00 Hourly
Job Overview
Assists residents in all aspects of their daily life as indicated in the resident’s care plan, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities.
Responsibilities and Duties
- Provide resident care to residents that include bathing, dressing, toileting, and feeding, as outlined in their care plan
- Respects the needs of team members and residents by adhering to schedule and documenting attendance properly.
- Maintain the safety, comfort, dignity, and confidentiality of all residents
- Encourages and escorts (when needed) residents to meals and activities
- Ensures all assigned residents are clean, neatly dressed, groomed, safe, and fed during assigned shifts
- Document all services provided in ALIS EHR for your assigned residents during your shift
- Monitors and reports any change of condition in a resident immediately to your supervisor
- Appropriately utilizes and keeps electronic documentation “devices” safe and secure within the community; efficiently documents care/services provided during the shift.
- Participates in and attends all in-service trainings as scheduled
- Ensures compliance with all Federal, State, and local regulations, as well as community policies and procedures
- Provides basic housekeeping and/or laundry services when needed
- May be asked to obtain weights and vitals of residents and document same per RN request
- Other duties as assigned by supervisor, RN, and/or ED
Qualifications
- Qualifications
- High School Diploma or GED. At least 1 year resident care experience required.
- Certified Nursing Assistant or Certified Home Health Aid
- Able to speak, read and write in English
- Knowledge and experience in working with the elderly population, preferred dementia experience
- Physical Abilities
- While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
- An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.

The following Q&A provides clarification regarding eligibility, scheduling, compensation, and expectations for participation in the Weekend Warrior Program:
1. Are Team Members (TMs) in the community eligible to enroll in the Weekend Warrior Program?
Yes. Team Members working in the community are eligible to transition into the Weekend Warrior Program if they wish to give up the weekday work schedule/commitment. This is a weekend only position.
2. What days are considered part of the Weekend Warrior schedule?
Weekend Warrior schedules are individualized based on the needs of each community. The designated schedule may include a combination of Thursday through Monday, depending on community staffing needs.
The program is not limited exclusively to Saturday and Sunday. For example, a Team Member who consistently works a Thursday–Monday schedule may qualify as a Weekend Warrior. This is based upon community needs. ED determines the need.
3. What are the rates for the Weekend Warrior Program for each role?
Weekend Warrior rates are individualized based on the community's established hourly rates for each role. The Weekend Warrior rate should be set at $1.00 more per hour than the applicable standard hourly rate.
4. If a Team Member currently works only weekends, are they considered a Weekend Warrior and eligible for the increased rate?
Team Members who consistently work the designated Weekend Warrior schedule may qualify for the program and associated rate increase. Communities should first identify Team Members who are regularly scheduled to work every weekend and determine whether their existing schedule meets the Weekend Warrior criteria.
5. What are the scheduling guidelines for the Weekend Warrior Program?
The Weekend Warrior Program is intended for Team Members who are hired and scheduled to work weekends, rather than Team Members who simply pick up additional weekend shifts or work EOW, as part of their regular schedule. Working EOW along with their weekday schedule does not qualify for this program. However, a Team Member who is hired to work Thursday through Monday every week could meet the intended scheduling structure for the program. Again, ED determines the community's need.
The program is therefore based on a consistent, recurring weekend-focused schedule, not by occasionally picking up one or more additional weekend shifts. Those that are hired to work weekends only , either every weekend or some commitment of routinely scheduled weekends.
