How you’ll make an impact in this role

- Coordinate the overall interdisciplinary plan of care across the continuum from admission to discharge, integrating patient/family preferences and values (LSW required, LCSW preferred).
- Monitor patient care through assessments, evaluations, and medical records, acting as a resource for adequate documentation and appropriateness of services.
- Advocate for necessary resources, the removal of barriers, and appropriate treatment options for post-discharge care.
- Maintain ongoing dialog with supervisors and other health providers to ensure the effective implementation of the health plan.

What minimum requirements you’ll need

Licensure / Certification / Registration:

- One or more of the following required: Clinical Counselor credentialed from the Illinois Department of Financial and Professional
Regulation obtained prior to hire date or job transfer date.
- Counselor credentialed from the Illinois Department of Financial and Professional Regulation
obtained prior to hire date or job transfer date.
- Social Worker specializing in Clinical Practice credentialed from the Illinois Department of
Financial and Professional Regulation obtained prior to hire date or job transfer date.
- Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.

Education:

- High School diploma equivalency with 2 years of cumulative experience OR Associate's
degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Additional Preferences:

- Previous in patient discharge experience needed
