How you’ll make an impact in this role

- Evaluate patient social and emotional needs by conducting comprehensive interviews and assessments to establish clear care priorities.
- Connect patients and families to vital community support by building strong relationships with local welfare, health and social agencies.
- Coordinate holistic discharge plans by collaborating with multidisciplinary teams, home care providers and community organizations to ensure safe transitions.
- Empower care continuity by consulting regularly with healthcare managers, statutory groups and family members to keep treatment plans aligned.

What minimum requirements you’ll need

Licensure / Certification / Registration:

- Licensed Social Worker credentialed from the Indiana Behavioral Health and Human Services Licensing Board obtained within 12 Months (1 year) of hire date or job transfer date required. Or Master of Social Work (LMSW) obtained within 12 months (1 year) of hire date or job transfer date.
- Drivers License preferred

Education:

- Bachelor's degree in Social Work required.

Work Experience:

- 3 years of experience preferred.

Additional Preferences:

- Previous experience working with Oncology patients preferred.
- Ability to demonstrate professionalism and compassion.
