Job Description
- Schedule : Friday 10:00 am - Monday 10:00 am
- Location : Remote
- Hospice coordinator for up to 8-10 RNs (target range but could vary) as well as other hospice nurses. Develops relationships with partnering disciplines (i.e. Supportive Services) and serves as a resource and offers support/guidance when indicated.
- Prepare and send out weekend prep sheet to keep team informed on weekend staffing and visit needs
- Process and coordinator intake and admissions over the weekend
- Serve as Clinical Leadership over the weekend
- Works in conjunction with other Clinical Coordinators to ensure consistent messaging and education for all Paradigm Health hospice clinicians.
- Oversees day-to-day clinical operations for associated RNs (including but not limited to morning call, questions from the field, visit coverage for staff call-ins and/or patient PRN visit needs).
- Maintains clinical logs (i.e. falls, infections, incidents) for applicable patients and brings data to QAPI. Identifies trends and executes improvement plans.
- Completes grievance follow-up promptly for associated patients to ensure service recovery. Documents all follow-up and re-education efforts.
- Oversees medication, DME, and supply utilization for associated patients to ensure patient needs are met while remaining fiscally responsible.
- Supports RNs and associated hospice care team with care plan meetings or challenging patient care conversations/situations when indicated.
- Oversees timely and accurate completion of HOPE measures and acts as the HOPE Administrator for applicable patients.
Requirements
- Active RN License in the state of Indiana
- 2 years Hospice Experience required
- Hospice Triage Experience required
- Knowledge of hospice regulations and COPs
- Valid state driver’s license and reliable automobile.
- CPR Certification
- Proof of current automobile insurance.
- Current health certificate/physical examination and TB testing.
- Criminal background check.
- Currently CHPN (Certified Hospice and Palliative Nurse) or willingness to obtain certification within first year of beginning new position as Clinical Coordinator.
