Essential Job Functions:

- Structures and maintains an efficient and effective referrals system with an accurate account of “active” patient referrals by priority across sites within AHS to ensure provider/clinician support, accuracy, and timely follow-through.

- Responsible for providing direction to referral staff and support to patients in navigating through the paperwork and processes involved with making and completing a successful referral.

- Ensures the timely processing, documentation, tracking, and communication of patient referrals, by the level of priority based on the provider’s communications and notes in the patient’s medical record.

- Maintains dynamic updates of any changes in referral requirements and networks with timely communications to referral staff, clinic staff, providers, and managers across AHS.

- Monitors analyze and troubleshoot timeliness of referral processes including cycle times and/or other relevant metrics.

- Provides regular and ad-hoc reports as needed.

- Along with QI Analyst, identifies and implements relevant QI projects.

- Collaborates with leadership to establish and maintain protocols.

- Performs quality control reviews and remediation of scanned data into the Electronic Health Record (EHR) system within established policies and procedures for entering records into the EHR system.

- Effectively prioritizes workflow to ensure that all patient data is entered within specified turnaround times.

- Assists the Director of Clinical Operations with the development and implementation of policy and procedures governing health information and records to ensure compliance with applicable specific federal, state regulatory and accreditation standards.

- Assists the Director of Clinical Operations in the development of tools, resources, and databases to accurately capture and interpret key metrics and data for quality reporting.

Supervisory Responsibilities:

- Oversees release of information requests, subpoenas, and other requests consistent with all applicable regulations and policies.

- Direct supervision of a department involving responsibility for results in terms of costs, methods, and personnel.

- Carries out supervisory responsibilities by AHS policies and applicable laws.

- Responsibilities include interviewing, hiring, coaching, and training employees

- Planning, assigning, and directing work; appraising performance

- Taking disciplinary action as may be needed; addressing complaints and resolving problems.

- Performs all duties and services in full compliance with AHS Excellence Standards.

General Agency/AHS Duties:

- Fosters an environment that promotes trust and cooperation among clients and staff.

- Enforces clinic policies and procedures, including maintenance of client confidentiality under HIPAA privacy rules, to ensure that the principles of AHS are implemented.

- Plays an active role in planning organization-wide activities such as AHS general membership meetings, fundraisers, special clinic days, and other events.

- Participates in general membership meetings, fundraisers, and other public events, as required.

- Participates in outreach activities, agency advocacy, and serves on ad hoc committees, as requested.

- Attends AHS staff retreats and Board of Directors meetings, as required.

- Participates in agency-wide quality improvement program processes and performance improvement teams to ensure excellence in the quality of services provided across the agency.

Preferred Qualifications:

- A Bachelor’s degree (B.A.) in public health, social work, health administration or related field

- A minimum of two years of relevant experience or any equivalent combination of education and experience.

- Prior supervisory experience

- Bilingual language ability considered advantageous to both staff and patients.

Knowledge, Skills, and Abilities:

- Basic working knowledge of medical terminology

- Working knowledge of “Universal Precautions,” demonstrates professionalism at all times.

- Thorough understanding of the importance of confidentiality and non-disclosure according to the general standards set forth by HIPAA.

- Demonstrated leadership skills with the ability to train and guide staff in the performance of their duties.

- Excellent verbal and written communication skills at the level necessary for understanding patients and provider or supervisor’s instructions, and for accurately documenting patients' medical information.

- Easy facility and adaptability to working with computers and software applications.

- Ability to effectively communicate with the patient population and staff while demonstrating a high degree of diplomacy and tact.

- Ability to work collaboratively with other AHS supervisors and managers with a focus on customer service and quality of care, exemplifying the core values and mission of the organization, always exercising utmost discretion, diplomacy and tact inpatient/staff interactions.

- Ability to multi-task and work effectively in a high-stress and fast-moving environment.

- Culturally sensitive and demonstrated ability and effectiveness in working with ethnically diverse populations.

- Willingness to work evenings and/or weekends.
