Who We Are:
Canadian Mental Health Association Durham is an integrated community mental health and primary care hub that has been providing mental health services to Durham Region for over 60 years.
During that time, it has evolved to provide a broad range of services to individuals and their families experiencing mental health challenges, from information and education to case management, psychosocial rehabilitation, housing and primary healthcare. These services are provided through inter-professional teams focused on providing clinically based services that, in addition to dealing with client mental health issues, address social determinants of health using the recovery model.
Overview of the Role:
Reporting to the Community Homes for Opportunity (CHO) Lead this individual monitors compliance and provides advice to the homeowners of the CHO programs within the Canadian Mental Health Association (CMHA) Durham.
Duties and Responsibilities:
Client Care & Support
- Reviews client intake and assessment information and prepares individualized, supportive Client Care Plans focused on long-term stability and growth
- Ensures clients understand CMHA Durham services and agree to service expectations through appropriate agreements
- Establishes supportive therapeutic relationships with clients and advocates for and with them
- Provides assistance to clients in establishing peer and family support networks
Compliance, Reporting & Documentation
- Develops, updates, and monitors all aspects of compliance for a designated group of homes in collaboration with the CHO Lead
- Provides summary compliance reports to the CHO Lead within approved timelines
- Documents contacts, summaries, and reports in a timely manner in accordance with CMHA policies and procedures
- Monitors the reporting system for serious incidents and resident complaints in collaboration with the CHO Lead
Program Coordination & Clinical Support
- Provides support and direction to internal clinical staff within the program
- Ensures resident care plans align with a recovery-oriented environment and meet clinical needs
- Ensures identified improvements are completed in a timely manner
- Monitors and updates the CHO waitlist
Stakeholder Collaboration & Liaison
- Acts as the first point of contact for a designated group of homeowners, providing advice and guidance as needed
- Acts as a liaison for homeowners and provides conflict resolution support for homeowners and residents as required
- Consults and collaborates with community resources and service providers to ensure clients access appropriate services and continuity of care
- Performs other duties as assigned by CHO management
Client Safety
Adhere to CMHA Durham client safety programs including program policies as well as infection control and prevention procedures. In partnership with clients, family members, and fellow staff, maintain open lines of communication to assist the organization in identifying and achieving quality outcomes for client safety. Report client safety incidents and suggested improvements to manager or delegate.
Other Duties as Assigned
- Attend, participate in team meetings and all employee meetings
- Participate in organizational quality improvement activities, e.g. accreditation projects, committee work, special events
- Participate in quality improvement activities at the team/program level
- Participate in external committees as required

- Develop strategic actions, develop/provide information on policies and procedures,
- Proofread, perform comprehensive assessments, and read documents,
- Resolve conflicts, problem solve, facilitate meetings, attend meetings, oversee daily operations, manage caseloads, prepare documents, listen to clients etc.
Risk Management:
Minimizes risks adhering to CMHA Durham Policies and Procedures identifying, assessing, evaluating and controlling risks ensuring well-being of employees and clients
Qualifications:
- Post-secondary degree in a relevant field (e.g. Bachelor’s Degree in Mental Health, Social Sciences and/or relevant discipline) and a minimum of 5 years clinical experience and/or equivalent combination of education, experience, and/or lived/life experiences is required
- Sound knowledge of the Mental Health Act, Health Care Consent Act, social service benefits, and related legislation in Ontario
- Excellent written and verbal communication skills
- Ability to develop and maintain positive relationships with clients and community partners, and represent the agency professionally at all times
- Ability to effectively acquire and utilize community resources to meet clients’ needs
- Flexibility to adapt to change
- Ability to work independently and in a dynamic team environment
- Strong critical and creative thinking skills
- Ability to work with individuals from diverse backgrounds
- Valid Ontario Driver’s license, access to a dependable vehicle in safe working order and appropriate insurance coverage
- Knowledge of MS Office (Word, Excel, PowerPoint, Access and Outlook
- Satisfactory, current Vulnerable Sector Check and satisfactory Professional Work References required
- Knowledge of Indigenous communities and cultural practices (preferred)
- Basic to Intermediate proficiency in French considered an asset
- Work outside of normal business hours as required in compliance with the Disconnect from Work Policy
Working Conditions:
- Works on site in office settings and off site in CHO Homes
- Exposure to abusive/irate clients, deadlines, multiple demands, distractions/Interruptions
- Little exposure to hazards
